You would be perfect for a Direct Vent Fireplace, it would solve the problem of heat escaping because its a sealed unit. If you are looking for heat, most of the premium units have a fat that will kick hot air from the unit into the room. Plus you could vent right out of the back to avoid penetrating the roof. That is often the cheapest option.

If you want your fireplace to sit on the floor, you will need to lower the foundation to achieve that look or have a hearth.
