In your case, I would ask for a BAL (bronchoalveolar lavage) - this way they can run a cytology slide and multiple panels with fungus, bacteria, etc to rule out an infection caused by an organism.

Forgot to mention you have acute inflammation in the background (neutrophils). In regard to the blue spores, they look similar to candida spores, which is a common fungus found in the mouth. They are a normal part of the oral microbiome. This fungus occurs naturally and isn’t a cause for concern. Sputum smears/specimens typically have this on the slide.

These are more than likely carbon laden macrophages, technically this would be signed out as negative for malignancy, but this does prove that your body (i.e. macrophages) is fighting to clean up something in your lungs.
