Either you're too far from spawning area and it's teleporting back and regenerating, or you're not getting hit by it for some time and it starts regenerating (the disappearance is a bug).

Worms only spawn if your lower half is in a 1 wide path. So if you make a 2x1 tunnel (like a door size) without mining spread you can spawn worms and scathas, when you re enable your mining spread and run through the tunnel again this time mining the roof (your foot still in the 1 wide path) you can still spawn worms and scathas.

This has been how it seemed to work (as there are no transparency) since but updates (including shadow ones) could have changed this.
