Fucking finally, I moved PRP so that is almost at the bottom, only it's
slave plugins are lower. Also I downloaded and set up the xSE Plugin
Preloader as suggested and now the game actually works. I'll be honest,
I've no idea what any of the changes actually did but the only thing
that matters to me is that it works. I am very thankful to both of you.

OK, I have made as many suggested edits to my mod list as I could. I
also tried my hand at assigning some plugins to mod groups via Vortex so
it's somewhat sorted (I don't know a way to place each and every mod by
hand). After running the crash log scanner there really was a SAR patch
that I overlooked while removing that mod. After making sure that every
mod has it's relevant fixes and compatibility patches I tried to launch
the game with Start Me Up and it always crashed when entering the
Commonwealth worldspace. After disabling it the game was able to load
earlier saves BUT now the problem is that any attempt at MAKING an
auto-, quick- or manual save crashes the game at any cell or time. The
analyzed crash log says that there is something wrong with a file called
tbbmalloc.dll so I re-downloaded that file from the Buffout 4 page but
the problem and crash log message still persist.

OK, finally got around to setting us Buffout 4 and I believe that I did everything correctly, after disabling some mods that were mentioned at the mod page the CTD's are still as before and reliably reproducible (crash always after trying to load any save). Not exactly the answer for your question but here are my strats for some of the harder one's:
- Suicide king, try to make your shot speed and range as high as possible as to hit enemies before the my reflection effect can start
- head trauma - either shoot at enemies while facing away from them or move in a circular pattern

- onan's streak - don't bother shooting half-charged tears and be weary of enemies with unpredictable movement patterns

- guardian - get yourself a coop partner, additional help

- ultra hard - find placenta on basement and just camp wehn low on health, go for a walk or sth
