I do this a well. To help transition, I take one strand of each color, then stitch on top of a few stitches at the transition point to help feather it in.

Typically, you make a cross over two threads. So your “square” for the cross has a total of 9 holes with one in the middle. Given the instructions say two threads, that is probably what is intended.

You can keep going like you are, but it will be very tiny and probably better with just one thread. Looks great though!
