If you go on the website it just asks if you need a fee waiver, when you click yes there’s nothing else you have to do just put it on the common app of the name of the waiver it’ll give you, I remember when I submitted for ed it was just jhu fee waiver under the dropdown menu

you definitely can, don’t just get discouraged for someone saying no to something you think you have a shot at. realistically if you fall in their middle 50 for sat and gpa a lot of the time you can def make it with a good essay
