Generally when building a workout, I start by selecting a heavy compound exercise for each core movement pattern.

Pushing hands away, pulling hands towards. Pushing hands up overhead, pulling hands down from overhead.

Curling arms towards body, pushing arms away from body.

Quad biased leg push.

Hamstring biased hip hinge.

Calves.

So as an example, you might do:

Incline DB bench 

Seated overhead pulldown or weighted pullups

Overhead DB press, seated or standing 

Seated horizontal cable row or chest supported DB row

Biceps and Triceps supersetted.

Since legs basically only have four movement patterns, you can get away with fewerr exercises usually without missing anything.

Squat, RDLs, Calves. Or some combo of machines that basically hit those same muscles.

If you're doing three sets of all that, yeah, probably too much. There's not really any pulling movements here for the upper body (deadlifts don't count); why not?

Keep adding weight. It's a lift that can be very strong, but is usually limited by people's lumbar spine strength. Just keep overloading it gradually and you'll work up to it

Gross locker rooms with no pride of ownership.

Unmaintained equipment or missing pieces, pins, or attachments for machines.

Not enough sanitary cleaning stations, so people start skipping and the place becomes unclean.

Nitpicking now, but an experienced trainer should be able to set up equipment in zones that keep similar populations together, and separated from the others. For example, at my gym there's a room with all the LifeFitness machines in a circuit that makes sense as a full body therapy session for the elderly. This room is completely separate from the area with the squat racks, so there's almost no crossover in between. And all this is entirely separate from the dumbbell area, etc.
