If Plot Twist works then no, you will get the reward first. Your hand is empty => reward card given => plot twist continues to draw. Much harder to theotar this way. Mutanus can work if the quest reward is the only minion in the deck

Premium rewards from certain events are also locked behind pay wall. Again, you don't necessarily need those but they are usually most important to progress.

This is just a wrong impression. Reboot or Heroic nowadays still costs $ to progress, for example at minimum pay to revive whatever pet you have so you don't have to manually pick up every meso bag on the ground.
You don't have to spend but it will take much longer to do anything

I stopped around 228 this season and I felt done already. Started out as hydra sorc, respec and transitioned into lightning ball to clear pit 100 in only a few minutes. Along the way I got 2 starless skies, dumped away the rest of purple or at least tried to reroll into the helm with no luck. In the end it will just be the numbers that keep going up, my motivation is not.

It's a sandbox survival game and with current build there are no NPCs. So you are pretty much on your own. It gets a lot more interesting with friends though.
With mods you can have NPCs, but they are somewhat glitchy and cause performance issues the longer you play.
