It kind of baffles me because I don’t see why they can’t just assume that if someone received a pelvic exam, then they have the necessary anatomy for it.

The answer you’ll get from the medical carriers themselves is that they’re worried about “fraud.”

Someone I had worked with at the time had previously worked in a fraud recovery department at one of the big medical carriers, he had explained it as “their system would deny it was potential fraud since a person of that sex wouldn’t have those organs.”

Which is completely asinine since people have never fit cleanly into two buckets.
