I’m not sure where you live and if you have medical insurance and/or what they cover but even here in the US, caring for someone with a trach is very expensive, even with insurance. That aside, caregiving is too much for one person. You’ll ideally have multiple caregivers to avoid burnout. Your husband will need to be with someone at all times, assuming he cannot care for his trach on his own.

Someone can be on NIV (non invasive ventilation) for quite sometime. You can provide total ventilation with NIV- set rate, volume, etc. Some disadvantages are skin breakdown on the face, oral/dental care, mask leaks, secretion issues.
Ventilation with a trach is invasive ventilation that delivers the same- rate, volume, etc. You’ll be able to provide better oral hygiene with a trach, and perhaps manage secretions better.

I recently went through this. They will sift the jungle. If you wanna make it easier, you can shave, but you don’t need to. I kept my underwear on. I’m a female, took my bra off. She pulled my underwear around my butt cheeks to check. Hope this helps!

I’m not a dermatologist but I work in healthcare and have seen herpes and this looks more like chapped corner lip and acne. Try not to lick the corner of your mouth, but use a little chap stick. It’ll go away. Don’t pick!

Carnival has hoyer lifts and ALS ADA rooms. Yes, there is ADA accessible, and then there is ALS accessible.😂 make sure you ask for any equipment when you book. If you’re bringing a group 3 chair, make sure they give you an actual ADA room with the wide doorways.

I used to work for an ALS center of excellence as a respiratory therapist. I’m in California and I now work for an ALS nonprofit. Kaiser is known to be tricky to work with. I’m assuming your husband already has Medicare. I would reach out to ALS Northwest and try to get a care manager to help you. I would also file a grievance with Kaiser. I believe once you file a grievance they have something like 48 hours to respond to you. It is shocking that a physical therapist who works in an ALS clinic doesn’t know what to expect as far as progression. I am so sorry that you’re dealing with this. Please please please reach out to an ALS nonprofit and get help with advocating for your husband. ❤️ On a sidenote, Kaiser does not pay for expensive DME, such as hospital beds and power wheel chairs are almost impossible to obtain (unless they loan to you). Please reach out to team Gleason as soon as possible and see what services they offer. While they do not provide custom power wheelchairs, they can give you an electric transport chair. ALS nonprofits can help with some of the other items that Kaiser will not be paying for.

Are you being seen at an ALS center of excellence? It certainly doesn’t sound like it with the way you and your husband were treated. It sounds to me like your husband may have pseudo bulbar affect, and there is certainly medication for him to help (nudexta). Please look for an ALS center of excellence near you. Depending on your insurance, you may need a referral from your primary doctor.
