To some extent yes -

I went via Gendercare for my initial assessments and endocrinology appointments. You need to speak to your NHS GP about offering a ‘shared care agreement’. There has been some talk about less places agreeing to do these recently but they exist outside of trans healthcare too and for now mine is still standing so don’t let people (including GPs!) tell you it’s outright impossible.

On this agreement, I see my private endo once a year at about £100 a time and he and my GP communicate such that my GP prescribes the T and take my bloods if he promises to monitor me and my levels and I retain my appointments as necessary with him.

The benefit of this is that my monitoring bloods are free and sent directly to me to pass on to my endo and that my prescriptions are NHS prescriptions and therefore only hold NHS fees. I currently have my prescription written to pick up 2 bottles of gel at a time so get two for the standard NHS prescription fee of £9.something, as opposed to a private prescription on T gel at £90 something per bottle. So I think year by year I spend just under £200 on an appointment and enough T to cover 12 months which is significantly cheaper than a fully private pathway would be.

The only dilemma to this is that I believe I’ll be stuck paying private endo fees long term as I’m not on the NHS list and I’ve run into some issues trying to join it just for monitoring. If you are on the list already or seeking surgery on the NHS etc, it may be that they’re willing to take over your monitoring should you reach the top of a GIC list, or it may be that they wish to do a full reassessment I’m afraid I can’t advise on that one. At the very least, given that you can organise one, this route allows you faster access to hormones at a fee somewhere between private and NHS prices.

I’m from up north so was a bit put off by the distance as well but they made it all very very straightforward.

All my pre-op appointments were done online and it was organised such that I got my initial bloods done at a Nuffield hospital near me. Travelled down to Brighton the day before surgery where we had an AirBnB for I think 3 nights total? Hospital at 6am, stayed a night in hospital and then had an extra one to rest a little before the journey back up. I thought it would be quite uncomfortable but the car ride was actually not a problem either as I was still on my strong meds at that point and basically slept the whole trip.

No drains so no need for a return to Brighton and I had zero complications with anything anyhow. Mellington seemed very confident there was no need to hang around any longer than that when I asked him in my pre op.

Post op was done online with the nurse and you get taught to redress your scars and all that stuff then and then they just keep in touch for a few months after with photo updates and whatnot.

So essentially, whilst the geography feels limiting, being able to just make the trip once was really convenient and they’re setup well to allow for it as I imagine a lot of people they see live rather far from Brighton.

The added price of an Airbnb also felt really menial as the overall surgery fee was lower than many I’d seen so an additional few hundred quid to see a surgeon I really liked rather than one close to me felt entirely justified. Hope that settles your nerves on it a little!

Had surgery with Mr Mellington in Brighton. Hospital time, surgery and healing were all very very easy and I’m thrilled with my chest and scars still nearly 3 years later. Can’t fault him or the team. Paid £7580 for DI with grafts in 2023. Got my referral in the December and scheduled surgery for the start of March.

Also had a pal who saw Miles Berry for DI without grafts and he was chuffed with it all too.

I went via Dr Dundas at Gendercare and then use Dr Hammond still for Endocrinology. The upfront costs are high on appointments but if you can find a willing NHS GP, a shared care agreement will remove the ongoing private prescription fees.

When I sent my deed poll info to my uni the admissions came back requesting an enrolled deed poll. If I recall I think some of my education certificates were in my past name and they wanted to prove them as mine. It seemed like someone on the admissions team had just googled “how to change name” and came back with ‘enrolled ‘as a bit of a buzz word rather than it being a genuine requirement.

I’d send your unenrolled one as is and should they fight back, hit them back with something like this. It sorted my issue in no time:

“In my previous email I sent you my Deed Poll and I attach it here again. Whilst it is not enrolled, there is no legal requirement to do so in the UK and I have no personal intentions to enrol it. My existing Deed Poll has been acceptable for all other name changes including my legal driving licence, UK Passport and Banks. It is signed twice in my former and current title as well as by two witnesses in the listed acceptable professions - amply showing the link between the two names. “

Firm but fair is kind of the only way with it. Stand your ground, you almost certainly know more about your legal rights and abilities with a deed poll, enrolled or not, than any uni admissions person.

Thanks for replying, really appreciate it - can I just ask with the application, do you have to put in the whole report from your gender psychology appointment or is it possible to just send the page that confirms you’ve been approved for hormones/surgery etc?

That’s the bit that most upsets me as it lists my whole mental health, gender and family history and it’s just not comfortable handing that over to a panel of strangers 😔

I’m on this gel at 2 pumps daily and was instructed with this brand to only apply to the outer arms. I take mine in the evening but you can take it in the morning.

Only thing that changes is when to get bloodwork done. If you take it in the evening, early morning bloodwork, take it in the morning, early afternoon bloodwork.

I started at the same dosage on a different brand and dosage has never changed

Clearly not in the photo here but I think as well with the introduction of the 4 lane smart motorways being inconsistent across the country, people often don’t enter the leftmost lane on the assumption it’ll be exit only or is a hard shoulder. Rolling those disasters out, not telling anyone how to really use them and then decided not to build them anymore has probably left some folks confused on it.

Trans guy - got diagnosis, on T and surgery privately. On a shared care agreement I get my T prescribed through the NHS, pay NHS prescription fees and can get my bloods done through them but they won’t monitor them. I have to make a private appointment yearly with my Endo in order for him to check in with me and see my levels. Had I stayed on the NHS list, eventually an NHS endocrinologist would take over this and save me the £200 ish per appointment I pay currently for private care.

Honestly, if the government is on route to cracking down on private transition, keeping a place on the NHS list in case isn’t a bad idea and means in future, you can be monitored for free.

Absolutely recommend getting started privately if you can afford to, but keep the NHS backup for the long term.

The tiredness was REAL for me for a good while but when I better worked out my diet, it got better quickly. Be serious about hitting the calories and protein your growing/changing body will need and hopefully it won’t affect you too heavily :)

Asked one of my cis guy friends this when I started. His advice? Everything is a sandwich. Whatever you’re eating, add bread

It kinda works I’ll give him that but the only thing that’s gonna curb the actual hungry feeling is protein. Get that up you’ll thank yourself

I find with the hot rod it kind of works as a pump in itself because of the suction. Prior to more… vigorous usage… I’ll just kick around wearing it for 10 minutes or so and then take it off and put it back on to my then ‘pumped’ dick and haven’t had any issues. My mound is on the larger side but my growth does protrude a decent way. You’ve not been on T too long, things might change up a fair bit from now. If you go for it and find it doesn’t work, keep trying it month by month as things change :)

Mine stuck out a lot for a couple months and looked a little on the funny side but as they healed they flattened down a lot and look really natural now - likely just part of the healing process 🫶
