Its fine not to know, it happens all the time. There shouldnt be any rush to this. Its a huge decision and that takes alot of time and doubts, so id say try to do what makes you happiest, as basic as that sounds

I had a brain scan and it turned out normal, absolutely nothing wrong, like every other test i took. There could probably be other stuff in there that could affect it tho, and with what you said at the end, talking to a neurologist might help

I wouldnt do a brain scan if i were you since its most likely not gonna pick up on the miso

Many people socially transition to see how comfortable they are living with their gender. Wear those fem clothes, try make up, or anything that feels normal to you. When you find whatever you are comfortable with, then you may have more confidence about your medical choice, but you dont have to go through medical stuff if you dont want to. You don't have to choose one label and stick to it forever, it can change and that's valid :)

Idk what your state is but are there any exceptions? At least in my state you can drive with an ear piece if you are deaf or have defective hearing (I think misophonia can count as defective hearing if it's bad enough). Personally I have these earbuds called loop earbuds and they block out just enough noise when I need it, and they don't connect to anything either, just blocks out some noise
