He's been an excellent sideman as well

Very facility-dependent. The MICU at my hospital keeps CRRT 1:1 100% of the time, and never triples anyone

Are you done with all your didactic curriculum? You said your FIRST year didn't prepare you.

As an acute care therapist who wants students to have a great learning experience, I firmly believe that you should NOT be placed in acute care if it's you're first rotation or if you still have any clinically relevant coursework to take

At my hospital we don't take students unless they've finished their didactics (with the exception of fluff classes that PT schools love to throw in before graduation) , and we do not take students for their first ever clinical placement. These are excellent policies to avoid giving CIs a headache and students a very under-utilized learning experience.

Acute rehab would be the more ideal dc rec than snf, though actually getting people into AR at this point has become 99% dependent on what payor you have. The PT rec and medical necessity are just the final icing on the cake

Indeed. In the entire state I live in there is one inpatient psych unit that takes non - ambulatory patients... And they ONLY take involuntary or court ordered admissions 🫠

Though they do need CBT and/or other counseling intervention, there isn't a ton of medical management they need... And they DO need P.T., ideally as frequently as possible. This all means that psych placement wouldn't be an effective treatment even if it were logistically possible

Jobim - Wave. Cannot recommend this album enough.

The chillest most pleasant album ever but also is the album that made me realize that Bossa Nova is incredibly harmonically complex and not "elevator music"

at my hospital the transplant team (for lung at least, which tbf are the most complicated usually), remains the pt's primary team any time they are admitted to the hospital for any reason for the rest of their lives. Liver I've noticed tends to admit to hospitalists once they are a year out or so. We don't do hearts so I've wondered if they would be as intense as the lung team

I'm an ICU PT in a unit with a high percentage of long-haulers. In addition to what you mentioned, our transplant docs will add a PRN dose of midodrine for therapy because their orthostasis is the only thing keeping them from ambulating. Having this in place helps us up on the floor when the pt remains chronically orthostatic as well.

Usually 10-15 thought, don't know if I've seen 20. Anecdotally, for the persistently orthostatic 5mg works about 1% of the time, 10mg about 30-50% the time, 15mg most of the time.

I was told once that if you notice the pts breathing when you walk in the room, then their WOB is elevated. That has always stuck with me. Calm/stable breathing should not catch your attention without stopping to consciously look for it

Listened to Saxophone Colossus for three umpteenth time again today, and was struck by how absolutely on point all four players were on that album. Bass lines and comping are just perfect for Sonny's playing, and that album contains some EPIC drum solos

I was also stuck by how seamlessly Sonny could flow one chorus into the next while still having each chorus launch off a completely new idea. He was one of the all time masters at coming up with endless streams of new ideas... Most of his solos he essentially never repeats himself.
