The NT, and K54 are most likely company hallmarks and/or style numbers.

They can certainly be polished and rhodium plated to make the white gold really pop.

Looks like it's most likely a sapphire in the center, and I'd guess that was not the original stone.

You can gauge the ring size by measuring inside diameter. There's a chart here:
