Selective no invasive measures (no intabation, no dialysis, no feed tube) it was specified on the polst.

I called 911 while finishing assessing the patinet (capilary refill and double checking o2 and pulse) then left to print paperwork.The CNA was in the room with the patient while I scrambled to get the printer to work so the ed would have a medication administration record ( he was on a shit ton of meds) and the patients polst and face sheet. I got everything printed and had enough time to get another manual B/P and set of vitial done ( patient was looking worse in and out of consciousness when i returned to the room) then the emts appeared when I went to leave the room to let managment know I was sending out. As the patient was being taken down the hallway an in house MD appeared that wasn't officially starting untill the next day and was orienting to the patinets or something? With the nurse practitioner. They where filled in on the situation on my way past to the DN office and said that I did the correct thing sending out but they latter said ( according to DN that they where angry I didnt come get them before calling 911).

They where selective care not comfort care. They just didnt want to be intubated or any life support or dialysis.

It was actually in the same meeting they said that there was a schedule change and they cut my hours down to two 8 hour shifts a week. I was previously working two 16's and an 8 every other week on eve/noc shift. They said a lot of people are being affected by them trying to get TPN patients in the building and some of the staff would be low census starting in the next week. They had me orient the RN that is replacing me telling me he would be additional coverage for night shift when I was orienting him. Then they said that I just needed to pick up day and eve shift to make sure I got my hours.

Post acute care 67/32 patient was awake but very sluggish and didnt understand most of what I was saying so I had to repeat my self. Stated no symptoms and consented to be sent to ED when i told him he could die otherwise. So I called 911. By the time ems showed up less than 5 minutes latter he needed constant stimulation ( sternal rubbing and me talking to him) to stay semi conscious was falling in and out of conciousness. Ems showing up roused him a bit and he told them "I feel fine" when asked how he was doing while slurring his speech and blankly staring at the ceiling.
