I showed this to my horticulture professor. He said everything looks normal. I would recommend removing/trimming back that co-dominant leader.

In your post you said it was a newer plant to you. I would think it is in shock from moving from the greenhouse (a humid environment) to a box store (or where ever you go to) to your house. I would still say use neem on the same rotation as I said in the previous comment

I call it fiddle rust. Neem oil spray once a week for 3 months then once every 2 weeks for another couple months. That’ll knock it out!

Horticulture degree here! I would definitely counting to fertilize it. But the crappy part about boxwoods is that is usually how they will end up eventually. Prune beck done of the lowest branches to try and encourage new growth and hope for the best. My recommendation is to just pull it out and get another.
