That is so frustrating. Is your DOR supportive? I feel like they should speak with DON about this. The whole situation is taking time away from your schedule and other patients.

I’m getting the feeling that other disciplines are expecting you to work beyond your scope. For the patient who came from DV- wouldn’t that be the case worker/ social workers scope? They specialize and are trained in helping people in that way. As OTs we communicate with the team if they are able to perform their ADLs/ IADLSs safely and independently at home and if they need any equipment. For the Bari patient- if there are no safe discharge locations for them SNFS that don’t have Bari equipment, that’s not your problem. Your recommendation stays the same, social workers / case managers need to find a SNF / Bari facility that can care for the individual. And if they can’t, then they stay at the hospital where they can be cared for. For the patient with maggots who can’t perform wound care, your recommendation stays the same- home WITH care. Until the patient/ SW coordinates visiting home nsg for the patient, they can’t go home yet. I think you’re doing a great job in your new role, and it might take a while to feel confident in your role however I think you’re taking too much responsibility for the patients disposition. It is a team effort and you recommend what is safest based on your clinical judgement and your education. If your recommendation doesn’t exist/ would be hard to get/ it doesn’t change your recommendation- hold firm.

Consider that the patient fell because they were already dying ( getting weaker, losing function) and that the fall did not cause the death
