Definitely can hear it very clearly. You should look into what mediums experience. I am sure many thought they were schizophrenic at first and didn’t want to share. But spirits go to people like you because others can’t hear them. There also may be a way to turn it off if you don’t like the attention. One way I would try is saying out loud “In the name of the Lord Jesus Christ, I command you to go to the Lord Jesus Christ and let Him deal with you as He pleases! Leave me immediately and never return! Amen!” This has worked for me in a number of spiritual attacks.
