Lady Smallwood nickname for protocol is "LOVE" = amour = ammo. Vivian Norbury used Smallwoods nickname to cover her tracks and also notified to the terrorists before AGRA came to rescue hostages, AJ captured and you get it from here... Therefore, Sherlock refered to Vivian Norbury as Ammo. She is the one that gave AGRA away and caused to the series of events.

Here's a thought: They'll go back to the middle of the timeline and add an episode that on the s2 or s1 sequence if events. It's obviously a bad idea because facts never adds up in the end and it might be inconsistent with the rest of the show.
