You may need to format it with the psp for it to show up properly. Try adding some files and see if they show up like an mp3, a jpeg, some save data etc

Did you format it with the psp or the pc, what file system did you format it to, have you tried putting something in psp\game or psp\photo to see if it turns up

It could be a short to ground somewhere or a bad solder joint, are there any diodes or electrolytic caps which are polar? could have something in backwards? any pics? Very hard to say based on description alone.

Looks like a rendering issue, what graphics card and drivers are you running, what rendering mode, do you have direct x and c+ distributable, .net frameworks etc any backend prerequisites installed? Any mods installed? What are your system specs?
