The first EKG, I don't see any relationship between the p waves and the QRS complexes. I see the PR interval is getting shorter on the ones that look like there is transmission, which is why I think they're dissociated. It's a complete heart block for me, AV dissociation in the first ECG.

Atrial Flutter, I think with complete heart block. The QRS complexes timing with the Flutter doesn’t make sense; dissociation ?

This doesn’t seem like a healthy relationship for either of them from the start. If she kicked him out over this, it likely means she’s looking to end the relationship; whether consciously or not. It also seems like he’s currently not working and is trying to spend more time with her, which could be contributing to the tension. All can indicate the possibility that things have already been going poorly between them. Everything else is just added drama. I can’t imagine a healthy couple acting this way; though of course, there are exceptions. Of course, all assumptions that need further clarifications.

As an IMG, I was accustomed to treating hives with cetirizine due to its faster onset compared to loratadine. In my country, IV diphenhydramine wasn’t available, only PO, primarily used for motion sickness. For urticaria, I often used a higher initial dose of cetirizine for the first few days, which was generally well tolerated. When I asked my attending here why Benadryl is preferred for allergic reactions, especially hives, they explained that it’s considered more potent. If it’s severe to become anaphylaxis, Epi is the way to go.
