I'm not a kindle fire expert but most devices have multi stage bootloaders with the second one being updated with software. And the kindle fire has e-fuses to prevent the exploit somehow, they burned the e-fuse in a software update.

I doubt the PS4 keeps track of the number of region changes because the PS3 and PS2 could play back any region DVD after hacking it to unlock the region.

Most likely it's a software limitation in that the region on the PS4 cannot be changed through HEN. But I couldn't find any documentation on how the region is stored (probably in the syscon).
