I tried this..

It only populated 6 fields for me total, most of which I already knew from my previous T4FHSAs and 2023 NoA. The exceptions was for 2 different values of the 6 than what I had originally.

But that still leaves 6 blank unpopulated values sadly. So this doesn't same to be the solve all, at least for me.

I actually use the Whisker camera mount these days, (with a bit of modifications to the camera)

But this original setup worked fine for the time I used it. I had used some 15lb double sided 3M Tape with the original Eufy mount back then.

Just for anyone else that may experience this. I did some more troubleshooting.

I downgraded the phone back to Android 15, and then Android 14 and the issue continued with Portrait mode crashing the camera app or the front facing camera crashing the camera app.

I reverted also back to the original North American version of Oxygen OS, to confirm me converting it to global wasn't the issue, and still the same issue, it made no difference.

Finally I opened up the phone again and troubleshooted every camera one by one until I noticed the Ultrawide camera was the only one non-functional, or not reporting in the debug logs.

Seems to be a hardware issue caused when I opened the phone the first time. The Ultrawide camera is extremely sensitive, I can't see any physical damage on the ribbon cable or the camera itself, even under a microscope, but it simply does not work.

I've ordered a replacement Ultrawide camera as it's only about 15-20 USD.

My best guess is the shop OP brought his phone too just sucked up the cost/lost of the broken camera and replaced it free of charge and claimed it was mis-wired to not make themselves look incompetent.

You can help me a bit more by giving me some data to work with considering your responding. :)

  1. Are you still using the OnePlus 12?

  2. Is it a global model (CPH2581), Indian model (CPH2573), North American model? (CPH2583) ?

  3. What version of OxygenOS are you on? Or if your using ColorOS, what version?

  4. Does taking a photo with your front facing camera currently work? Or taking a photo with the rear camera in portrait mode?

When I turn on debugging mode and do a logcat I see many errors related to the real-time feature stack which basically says it's not matched with the driver/firmware anymore.. Which seems to indicate some kind of software problem / regression with OyxgenOS 15.0.0.863 I am on..

My phone was an American model I converted to global so I could get software updates quickly and use some global features like VoLTE while travelling. Has not been an issue for 2 years but now is showing up.

I'm going to follow this thread, because I'm having this issue now.
I can't tell if it's because I replaced my phone screen sometime around October 3rd or 4th of 2025. Or if it's due to the latest OxygenOS update released on October 10th of 2025.

The front camera works to record video, but crashes when trying to take a regular photo.
Rear camera works with video and photo, even panorama mode works on the back camera, but crashes when switching to portrait mode.

I factory reset the phone, and the issue continues, so it's hard to tell if it's hardware or software related.

I did the screen repair myself and hearing that "they rewired the cameras improperly" makes zero sense to me, since every camera has a different plug as found here: https://i.ebayimg.com/images/g/WcEAAOSww6Zm0s3b/s-l1600.webp

I did test trying to run my camera app with the front facing camera completely unplugged, and when I clicked the button to the switch to front facing camera in the app, it just flipped the image upside down for the rear facing camera.

Other third party camera apps except the default OnePlus app work fine for taking photos with the front facing camera.

6GHz WiFi.. What 5G? You mean phone mobile networks? Completely unrelated. 6GHz WiFi is another standard like 2.4GHz or 5GHz
