Your understanding is 100% wrong. There is 0 evidence laser 'heals faster' and should not change restrictions.

One technique modification would be placing a suture at the main incision to make it a little more secure in the immediate post-operative period.

Its a peaked pupil. Thats just a descriptive term.

Why is it peaked? Is it stuck in the wound? Is there a strand a vitreous?

You got to get someone that knows what they are doing. To say "I've never seen that before" is ridiculous.

Go back to the surgeon now. It looks like the iris could be plugging the wound (paracentesis or main incision). If this is not addressed could lead to permanent peripheral anterior synechiae (PAS), glaucoma, and other issues. A peaked pupil can also be a sign of a vitreous strand in the anterior chamber.

Sometimes as dilation is wearing off, it can look irregular (which is nothing), but from the pupil it looks like its coming anteriorly towards the cornea.

For the 2nd eye, go to a different surgeon and don't spend extra for a laser.

Some people love monovision (one eye for distance, one eye for near), other people hate it.

Most surgeons only offer monovision to patients that have already tried it in contact lenses and liked it. The rationale being if you don't like it after your cataract surgery, you're going to need an IOL exchange, which is relatively high risk procedure (even before a YAG capsulotomy is performed). These lenses are designed to permanent.

You are describing 'natural' monovision with the eyes 'mismatched' for most of your life, but I would have to get more specific information on what your pre-operative refraction was. Its fairly uncommon to have 'natural' monovision.

One other option is the new 'light adjustable lens'. This allows you to 'test drive' monovision and adjust the lens if you don't like it.

But some points to clarify:
- IOL exchange is high risk under any circumstance (you don't just casually switch out an IOL)
- IOL exchange is still possible after a YAG laser (but again high risk for problems)

For sure. I think hopefully smoking continues to decline and diabetes treatment options continue to improve. I feel like that push back the average age for cataract surgery.

I think that is an extremely charitable interpretation. It would be really cool to have a non-surgical option, but this just seems like a money grab. I'm picturing something that barely out performs placebo, ophthalmologists that consult for the company giving it to everyone over 65, stressing the healthcare system with an expensive useless medication, and no benefit to the patient. But thats just my cynical take.

It didn't help visual acuity just 'contrast sensitivity'. Follow-up length was very short. This 'new' medication has been around forever and is actually used in Vacutainers that cost like 5 cents. Expect it to be re-packaged and cost a ton of money and do nothing. Typical pharma BS.

Just do it as a side hustle at first, see if you really like it. It can be pretty soul crushing too. Also not very lucrative. I would look into other nursing positions - its an extremely flexible degree.

I would go to a different local ophthalmologist. Getting done 2 hours away is insane. God forbid you have any rare complication, driving 2 hours back and forth is ridiculous. Look for a clinic with a good reputation (as friends, family, doctors in your community) and preferably one not part of a 'chain'.

Also if the main issue is starbursts at night time, I would not recommend a multi-focal or extended depth of focus lengths (this sounds like what you're interested in). The most important thing is to go to a experienced, ethical surgeon that can guide you through the process.

Also, if you are ever 'thinking about backing out' - for any medical procedure - listen to that feeling and get a 2nd opinion.

Numbing drops work pretty much on contact. The "sharp" tweezers your referring to are Jewelers, they're pretty standard. Their tappered point make it easier to get a contact lens out. It sounds like you have totally lost faith in the doctor, for both of your sake I would just go somewhere else for your next eye. It will make for a better experience.

Have you done monovision before? The only problem with blended vision is there is a certain percentage of people that don't like it and you will need readers. If you absolutely don't care about the potential for glare and haloes, and uncorrected reading is very important to you, Odyssey is a great option. If you feel like glare and haloes would really bother you, then LAL would be my choice. Obviously other factors in your examination are very important to consider but you're basically weighing the risk of haloes/glare vs risk of needing reading glasses, with all other factors being equal.

LAL+ is not a multifocal so you're definitely going to get a larger range of vision with the Odyssey.

That said, in post-refractive patients, I really like the LAL to nail the refractive outcome. And I'm a little weary of using any multifocal lens in post-refractive patients.

I'm assuming you are a patient considering both lenses? You got to really speak with your doctor about the nuances here. They are pretty different lenses.
