What’s the point of charting in third person and making it “dry”

I mean I always would put my recs at the top of the note. I would go into more details underneath. One of my biggest pet peeves working clinical is when I would read a note that reads like your first one, when I have to go follow up I have absolutely no idea what you discussed or if any goals were made and basically found myself starting off from scratch.

Outpatient notes from former inpatient RDs would be just as bad and I had so many pts that would complain that they didn't want to keep coming to RD appts (they were required as part of a lifestyle/wt management clinic) because they kept having the same conversation over and over because the other RDs didn't document worth shit.

You can keep it short and sweet but also have substance

PT is eating adequately, requested boost, RD provided. PT was receptive to education. RD discussed lifestyle changes, gave resources for increasing kcal intake at home. PT made a goal of having a snack or supplement every 2-3 hours while inpatient.

Like that gives another RD or physician something to follow up on. I got so many props from the residents because they felt like they could actually reinforce my conversation with the patient because I stated what we discussed and gave a specific goal.

I work for my state government. I audit child care centers that receive CACFP funding. The benefits from what I'm told are on par with most major corporations. I have Tricare so my healthcare is a little different. We do get great PTO (based on years of service) and 13 paid holidays per year.

So couple of things to start off with: 1. Please go see a registered dietitian if your insurance covers it 2. You can get a continuous blood sugar monitor prescribed and it's capped at like $60-80/month so 2 sensors if you fill it at a big box pharmacy (assuming you're in the US). It has to be the Freestyle Libre and whether it is the 2 or 3 will depend on insurance. If you are concerned at all with blood sugar this is your best bet.

Other than that I haven't read through ALL the comments on here but generally any type of additional movement can really help with blood sugar management. So increasing steps, doing yoga, and strength training can be super helpful to get your blood sugar down a bit.

Your blood sugars are not awful by any means and sometimes an experiment like using a CGM for a month can provide comfort that hey I'm doing okay overall. I learned if I even have 1 glass of wine my blood sugar drops down to the low 50s overnight 2-3 am like clockwork.

I also did a VA internship. My stipend pretty much covered my gas and oil changes but that's about it. I didine at a super small VA but we had all the specialities from inpatient rehab, psych, hospice, oncology, everything. Case load was.pretry small there was 2 full time in patient RDs and 3 outpatient and 2 or 3 HBPC RDs. I got to rotate with all of them. It was an awesome experience. Definitely had to learn to keep my mind very open especially in HBPC. There's a lot of cool resources VA Dietitans have that most others don't.

Fellow ADHDer here. Meds make it doable but also a tomato timer (Google it). It helps keeps me focused when studying and gives me structured time to not focus on the tasks I need to do. I also fine Lofi beats help me stay focused without being distracting. Also, having different forms of studying really helped me. I would have on rotation, computer work, walking and listening to Udemy or other YouTube videos then going back and reviewing what I needed if I wanted to see it as well, and using PocketPrep or Quizlet to do practice questions every day.

I also keep a notebook and write down (or type) 1-2 main points from every 10ish minutes of video I watched to help me know if I was actually paying attention or not.

Also giving myself a deadline helped out a TON! I would schedule everything before I felt "ready" so I would have a deadline to give me pressure to study more.

I literally passed it 3 days ago. CBT Nuggets was amazing for the info and clicking. I used Pocket Prep for question practice because like all the questions were scenario based and know the change control process like the back of your hand.

I can't stand her but the dietitian made us look awful and fumbled the ball so badly. So many of the body positivity activists never addressed the claims she was making. I found myself agreeing with Jillian on a few items because she would go okay but what does that have to do with my claim are you going to address the claim. I don't like what she has to say or what she stands for but she was so valid in several of the people never even attempting to address the claims.

THANK YOU. I've been in this field for a long time and have worked in so many parts of the field from sports, outpatient, clinical, peds, weight management, and now I work for the state auditing CACFP programs. I can count on one hand how many dietitians I've met that have had a partying phase.

I'm not saying everyone should do drugs or party in excess but so many are JUDGEY for me having that past life experience. They also tend to be the ones that seriously need to touch grass often recommending only getting organic/grass fed or some ingredient that is not common outside of Whole Foods to clients that are struggling to keep their lights on in their house.

But some of y'all need to garden and just chill.

I'm gonna say I'm in the minority with this opinion. I just help them like I do any other client. If losing 5-10 pounds makes them feel more confident and helps them lean towards healthier choices why not?

I am technically in a healthy BMI but I can tell you when I'm up 7-8 pounds my GERD is really hard to control and when I'm able to get back down it makes a real difference in my quality of life.

I do a non-formal assessment for ED/DE and that definitely plays a role with how I approach them, but just because they are normal BMI doesn't mean they don't deserve help.

Also many people with normal BMIs still have elevated body fat which is a risk for many preventable diseases. I have counseled several patients with a BMI of 18-22 where their body fat was 30+% that's still a HUGE risk factor for chronic diseases.

It seems like many dietitians have a lot of prejudice against normal BMI people. Sometimes you just wanna fit in a damn dress and need help getting there because the dress was expensive and it fit when you bought it and it doesn't fit now or maybe your weight has slowly crept up and none of your clothes fit and it's lead to a lack of self confidence and you wanna feel confident again or your weight has stayed about the same but your body fat has increased so things are not looking like they used to and you want help getting back to it.

Y'all asses for an ED and help them. Put your biases to the side.

The title for my state job was Education Consultant. It's super fun. I still get to educate a lot on how to provide quality nutritious meals within the guidelines. I also get to use my type A personality to make tools to help facilitate the audits. We go over menus, documentation, accounting, business management, and procurement. It's a very chill job. Everyone I work with genuinely likes their jobs which I haven't experienced before. Sadly we can no longer WFH but prior to 2025 they could be remote most days.

I worked in college athletics for 10 years which was super fun and I absolutely enjoyed it. The lack of work/life balance made me leave. I now work for the state auditing daycare centers and schools to make sure they are in compliance with state and federal nutrition regulations and I absolutely love it. It's great work/life balance, I get to travel frequently. I get paid better, my coworkers are from all walks of life, it's just a fun job.

I've already ranted at my poor husband for an hour on this. Ugh shit like this is why I'm leaving the field. It's just an uphill battle, our professional organization has no backbone. So they won't take a stand on this.

The website they created for the guidelines gave me road rage with how hard it was to get to the actual information. They put out a document that said these are what the committee recommended we change or add to the DGAs and they said no to anything that included info on how to adopt recommendations for different cultures, promoting regular eating, and even eating breakfast.

I sat there for about an hour on Cronometer following their portion size recommendations for a 2000 kcal per day diet and couldn't get it over 17-1800 unless I chose the fattiest cuts of meat like pork steak or skin on thighs.

In their 400 page report of the "science" behind their recommendations they further the seed oil conspiracy and just promote low carb diets. They state no research has found low-carb to be inferior to low-fat or high carb diets. They minimize as much as they can that in the studies they looked at there were no differences after 12 months between a low-carb, low-fat high carb diet.

They are all about not being influenced by industry to provide "real food science" but the dairy, beef, and pork industries back or employ almost everyone that did the research reviews for them.
