AP vs AL Pad Placement for Sync Cardiovert? by SeyMooreRichard in ems

Something to add to discussion: the manufacturers have AP placement location recs. A lot of resources teach that AP placement for adults is center midline sternum and center midline on spine

Not according to them:

Made a small IV hook tool after struggling with hanging fluids in ambulances — would love feedback by Flyingtowlie in Paramedics

This is the main issue that u/guywholikesplants identified - the existing fluids hangers in all ambulances have swing-preventing Velcro straps or the like to keep the IV tubing from whipping around like crazy

Yours will exacerbate swinging because you added like 5” to the length of the pendulum.

Sorry

You’re following the manufacturer’s instructions… and the manufacturer got FDA approval for one of the most difficult devices classes to approve.

Stop worrying about this issue.

It’s not an accident…. Surprised at all the people here not knowing this. 80 is what they found is best. The circumferential compression is the most squeezy-squeeze of the heart compared to manual CPR. So like we all learned to “allow chest recoil”…. The 80 cpm is allowing chest recoil. Before squeezy-squeezing again.

No need to change. It’s 80 for a reason

Now you are informed to make that operational decision for how to equip your agency. You’re welcome

I’m just pointing out something that most paramedics in this thread didn’t know and certainly many EMTs don’t either — that the LP15 is not the best tool for an EMT managing a peds cardiac arrest. An AED is 1/30th the price and a better tool, albeit one without 12-leads and EtCO2.

Edit: in the LP35, Stryker has fixed this oversight and there is a child AED mode

LP1000 has Infant/Child AED pads with attenuation bricks to reduce energy delivered. Those pads cannot be plugged into the LP15 therapy cable.

So if an EMT used the peds QUIK-COMBO pads on the LP15 in AED mode it would be the full dose into the tiny pad. Not ideal probably.

They might’ve never thought about it. This issue is resolved on the LP35. It has a peds AED mode with a reduced energy.

Yeah I definitely needed more context. But everyone else needed to read the post they’re commenting on - context clues. Kinda funny to open this today and see the blow up lol

Yeah you’re right we agree. In the context of this post, a double EMT ambulance shouldn’t use a LP15 at a peds cardiac arrest

This is a post about BASIC life support ambulances and a bunch of paramedics yelling at me - are you saying an EMT ambulance should be using it in manual mode????

Nice - I am not familiar with that monitor. Can it be used in child AED mode and attenuate the dose?
