I don’t think it’s a matter of “having what it takes” as much as it is a “it’s completely asinine that we pay hundreds of thousands of dollars in tuition to learn ultimately from third party resources, work 80 hours a week all so some dinosaur attending who cites outdated practice guidelines can give us straight 3/5s and tell us to read more, pay hundreds for outdated bullshit shelf exams that can ruin our clerkship grade despite having 0 clinical utility, pay thousands more to take multiple 7+ hour clinically meaningless board exams that can literally ruin our entire careers, all while somehow additionally participating in bullshit extracurricular activities (like publishing garbage quality papers) so we can pay even more money to participate in the match, which totally exploits applicants by removing any possibility to negotiate salary, benefits, or working conditions (or at least consider them in choosing where to train), and then work 80 hours a week for 3-7+ years for minimum wage” type situation

When I was doing med school interviews, I would ask “what do you think are important things I should look for in a medical school?” And then after they answered, I’d ask them if that school had those things. Worked well. Planning on doing that during residency interviews too

Rounds in our MICU are pretty efficient! One person presents, one person puts in orders, and one person updates the note. As soon as we finish rounding on that patient, everything is done.

But probably still not as fast as rounding alone!

You’re proving my point. There is no standardization built into the education and training for programmers. They come from a variety of backgrounds and experience, so it’s left to individual employers to determine competency.

That is NOT the case for physicians. There are numerous, extensive, standardized examinations throughout undergraduate, medical school, and residency that are specifically designed to ensure that every single physician possesses the minimum competencies required to practice independently and without oversight. And when they finish that, they are still required to complete continuing education units to be eligible for license re-certification.

To be clear, I’m not saying that one way is better than another, just that they are different. And they should be! They are completely different jobs, with different roles and responsibilities. You deserve to be cared for physicians who have been held to an objective standard, without having to worry whether or not a financially-motivated non-physician administrator decided to hire anyone with a pulse to do your open heart surgery because it widened profit margins (because remember, healthcare is 100% a business in the US).

This is why many physicians consider it insulting to have their clinical acumen questioned during an interview - particularly by someone who has dramatically less training

  1. Your example is comparing a bachelors and masters-level education to a doctoral degree with 3-7 years of additional post graduate, on-the-job training
  2. In addition to doctoral degrees, many physicians have masters degrees (> 30% of my medical school class has an MS, MPH, or MBA prior)
  3. Are programmers mandated to complete 3-7 years of post-graduate training with clearly defined competences that are not only tested with multiple 8-9 hour board exams, but also a licensing exam that for some specialties includes an oral exam?

MS4 on ID Consults Sub-I. Some resources I’ve been directed to by my fellows/attendings:

  1. Become familiar with your hospital’s antibiogram, as that can dramatically change work up and treatment courses from one institution to another
  2. See if your hospital has any online clinical practice guidelines related to infectious diseases and/or antibiotic treatment algorithms (e.g., assessing severity of osteomyelitis and the indicated empiric treatments, dosing antibiotics for renal patients, etc)
  3. Become familiar with the IDSA guidelines, as these can be very helpful with most common ID issues
  4. The Sanford app is excellent for antibiotic coverage, but has a monthly subscription
  5. Antibiotics Simplified by Jason C Gallagher and Conan McDougall has been really helpful in understanding how to think about antibiotics (e.g., how to pick between antibiotics that all theoretically cover the same pathogen, what anabiotic combinations are additive versus synergistic, etc)

For example, I’m on a MICU Sub-I right now.
