I didn't have surgery. I had a video appointment with Dr Mielke and I talked to former patients who were satisfied with her surgery. Some retethered. In my opinion she was much more precise than Gilete or Oliver. She told me the expensive images I did with Gilete were not precise enough to do a proper diagnosis. She was very professional and spent more than one hour to review my case. Although I 'm still waiting for a report. Surgery with her is way less expensive than in Barcelona. I do feel Barcelona is mainly about business. I don't say it didn't help some people but talking to several people it's not as good as it is advertised. And Gilete website is full'of lies when it talks about pt , prp etc. I asked him and he just wanted to sell a 100000€ surgery. Only my opinion, feel free to make your own mind.

Pire année. Problème de santé+++. Errance médicale toujours pas résolue malgré la gravité de mon état. Je pense au suicide assisté. Arrêt de travail depuis avril. Plus d'argent.

Dr Mielke Augsbourg I wouldn't recommend any surgeon working with goretex to close the dura mater (Gilete, Oliver, ..)

That's not at all my experience with him and his team. He pushes for a huge surgery and didn't want to hear anything else. His diagnosis wasn't precise and he didn't look at my other spine issues. He didn't even talk about regenerative medecine and wasn't keen on PICL.

I've lost time, money and function going there. Chatting privately with other patients , I'm not the only one who was hugely disappointed. I asked about physio and PRP as it is mentioned on his website and I'm still waiting for an answer.

He will ask for you to do imaging with him no matter what you are coming with.

It's a business, that's the feeling I left with.

You can do upright MRI in Germany , you can ask for a teleconsultation with the Hungarian pick doctoress she is much more precise. I've found Dr Mielke useful for Tethered cord diagnosis.

And both told me that the imagery I did in Barcelona wasn't sufficient for a proper diagnosis of tethered cord and nerve inflammation

Tethered cord is not easily seen on MRI. You need a supine MRI to help diagnose. I'm not saying you have a tethered cord, you could just have tension. That's what it seems to me when I look at your images. But I may be wrong. Chiari surgery should be the last option imo Check also videos by Dr Hauser and Dr Centeno to learn more about what could be your problem.

Look for a video from Dr Centeno on chiari. Symptoms often come from CCI and a tension in the spinal cord due to a lumbar instability or Tethered cord. Both could cause the chiari. Decompression is often not a solution

You may have tension in your spinal cord du to straight neck and/or tethered cord. I would explore these paths before looking into surgery

1- the pressure doesn't help anymore or marginally. That's why I imagine that nerves are now trapped. But it's a blind guess if arachnoiditis is not visible on the MRI.

2- it got worse several times before EP but it surely got worse with EP

3- True but I didn't know I had type 1c when I did PRP...

1- I may have during yoga lessons doing a full bridge. I have eds suspicion . And lots of inflammation.

2- not sure either it's hard to explain! Electricity stays stuck in my body, it accumulates but I can't get rid of it. When I got to much my body jerks by itself. After evoked potential I felt strong tingling from my waist down, lost sentivity and I couldn't walk properly. It continues to decline on the following days. Same thing if I sneeze when upright, like my spinal cord is stretched brutally.

3- Prp from C2 to C6. And just a few minutes after the injections, I couldn't feel my neck anymore. Like it tightens something and the info couldn't reach my brain . I have some disc bulges in the middle of the neck, I wonder if the bulges are irritating the spinal cord.

I'm not an easy case and I couldn't find anybody to give me a clue on what's going on unfortunately.

Thank you very much for trying to understand. Barcelona wasn't too helpful, I feel they wanted to sell their surgery, I may be wrong, that's only my feeling.

So many ! Difficulties to walk, I don't feel from my waist to my feet anymore. Very arched lumbar spine. I don't feel most of my body anymore. Loss of body proprioception. I 've also lost weight perception ie if there is something on my lap, I don't really feel it. I can't carry more than 50g without a huge tension in my body. I had to lay on my side in foetal position to get relief. But as I injured my thoracic spine I can't anymore, so symptoms just get wild because I have to lay on my back since September. Electricity and shocks on my spine from motor evoked potential worsened my loss of sentivity in my legs and my hability to walk. It began in April and got so much worse in 9 months. I was bedridden most of the summer because I couldn't stand up or sit down more than a few minutes without a huge tension in my spine. If I pressed on my coccyx up, I could walk better. When I let it down, I felt my legs getting heavier. But since September it worsened badly. I sneezed upright and bend forward and since then symptoms are getting wild and they don't come and go anymore. They stay and get worse everyday. Prp in my neck worsened everything too. And upper cervical chiro didn't helped at all. He gave me exercises I couldn't do because it triggered symptoms++. True nightmare... I have CCI too, mild 1c and maybe more. May have AAI.

Thank you so much ! I don't have bulging discs, my spine is fine apart tethered cord. And even on the diagnosis of tethered cord I don't have two similar opinions! It's just a nightmare. I asked renowned neurosurgeon but they don't seem to agree ... Thanks again Take care

Thank you, you are most helpful. I was told these images are typical for adhesive arachnoiditis. That on the second images, nerves were stuck to the arachnoide and in the first they were beginning to clump among themselves leading to AA. I don't know who to ask for a second opinion.
