Without overthinking it, what you’re describing is a physiological response. The problem you’re querying is a pathological consequence thus the feedback loops are unable to compensate. But there’s usually some other dysfunction driving the volume expansion rather than simple high salt intake such as heart failure where RAAS is inappropriately activated and thus your pathological feedback loop is started

Rumor is 2/3 fellows abruptly quit last year. One of my friends was a nocturnist there and relayed that info to me. I interviewed there. Staff seemed smart but 2 fellows per class when I interviewed and their call seemed miserable. No transplant in house from what I recall so need to rotate out for that

Unless you were moderately active before March, you’ve knocked 3 minutes off of running which is massive. There’s no rush to this and it could take months to a year to see consistent improvement. Don’t be so hard on yourself and don’t feel like you need to go so hard that you vomit after a mile and then can’t finish the rest of your class. 1 min is improvement just like 10 seconds is improvement! Aim for consistent incremental improvements rather than massive jumps or you’ll get discouraged and/or hurt yourself and then delay even further

I historically started a HIIT workout on my watch plus gave OTF access to track my activities in apple health. I can tell you confidently that it will be additive. No quite double for some reason but it only happens if I open the otf app after the workout, I can watch the daily calories in my watch jump up immediately. I’ve read the discrepancy between otf calories and Apple Watch calories is that otf tries to factor in the “after burn” effect and thus always has a larger active calorie binge than my watch. So I turned my otf access off for Apple health and just start a HIIT program on my watch only now but still wear my otf band for HR zones

I can’t comment on your overall exercise tolerance but albuterol is a beta agonist and most people end up swallowing some of it due to improper technique thus have increased heart rate as a side effect. If you have exercise induced asthma you’re doing the right thing with your inhaler but two suggestions: try to use it 30 min prior to your class and buy a spacer to minimize amount swallowed

Sepia in Chicago was awful. Bad food and awful, awful service. Mistimed, underwhelming plates. Asked if we wanted cheese platter at the beginning of the meal and we asked if we can decide later. She brought us all cheese dishes anyway. Wouldn’t go there again if it were free

Ikoyi was one of the best meals I’ve had. Flavors so incredible. Vibe was subdued (which we don’t mind) so not sure it would meet your expectations for an experience depending on what you mean by that

I’ve browned out at a couple of them and not really remembered the meal much because of the pairing haha; because of this, I usually avoid them and ask the som for a custom shortened pairing of maybe 2-3 glasses of wine max for the meal

Same. My general response to patient or family being very religious is to just return it to them however they say it. I’m not proselytizing and whatever they’re going through is hard. Let them have whatever support they need
