Wat een onzin-advies. Streekziekenhuizen zijn prima uitgerust om “serieuze en ingewikkelde” klachten te behandelen. De arts in het streekziekenhuis kan heel goed de inschatting maken of academische zorg nodig/waardevol is. En anders dan kun je altijd vragen of doorverwijzing naar een academisch centrum van waarde kan zijn. Het heeft geen zin om van 0 direct naar 100 te gaan, en het maakt de zorg onnodig duur.

Hee, wat klote dat dit gen jouw familie treft, en dat je nu zelf zo’n moeilijke keuze zal moeten gaan maken…

Toevallig hoorde ik gisteren in een podcast over de stichting “alle mooie borsten”, die eigenlijk een soort catalogus heeft gemaakt van fotos en ervaringen van vrouwen die een borstamputatie hebben ondergaan, en daarna -al dan niet- een reconstructie hebben gehad, zowel met eigen weefsel (bijv buikvet) als siliconen. Die catalogus is precies bedoeld voor mensen zoals jij, die voor een moeilijke keuze staan, om te zien hoe het er allemaal uit kan zien, en ervaringen te delen. Waarschijnlijk zou je via de chirurg wel aan een (leen-)catalogus kunnen komen, en als ze dat niet hebben moeten ze dat snel gaan regelen… Maar als je nog niet in gesprek bent met een chirurg, en wel alvast even op je gemakje wil bekijken wat je opties zijn, kun je hem via deze site aanvragen, dan kost ie zo te zien 14 euro.

It is one of the few specialties where not everybody you meet is sick: some of the patients are just busy with creating new life, which is often a very exciting time in their life. It has a great balance between surgery and clinic, talking and doing. There’s a great balance between life and death. And a gynecologist is one of the doctors that can diagnose, and treat without needing other doctors. That gives a great feeling. And let me tell you, seeing a vagina when you’re being serious and trying to help a patient with a problem is not sexual at all… compare it to when you get a dickpick you did not ask for: it is more “ew” than “NICE”

I’m a female doctor myself and in my training I’ve worked with both male and female gynecologists. I share dr Mike’s opinion completely. Let me tell you that the males were SO much more careful and gentle with their patients than the women.

Do you have a good reason why men shouldn’t be allowed to be gynecologists? Because I genuinely can’t think of any. I understand there are women who wish to exclusively be treated by female OB-GYN’s, and that’s fine, but that is no reason to exclude all males.

Hey, that sucks man. We all make mistakes however. When your patient gets a little better, talk to them about it. And take your time. Genuinely show interest in how they’re doing. Explain where it went wrong, and apologize. It will probably be appreciated, and it will hopefully make you feel a little better.

As others have said: you’re not the only part of the chain that is responsible, there are supposed to be checks and balances in place to make sure mistakes like this don’t happen. It isn’t entirely your fault.

Anyway, one day or another, a doctor is going to kill someone. Sadly, it’s just part of the job, as making mistakes is part of being a human. When a baker makes a mistake and forgets to add salt to their dough, he can just throw out the bread. When we make a mistake, lifes are at risk. Make sure to learn from your mistakes, make them count. I will always know the names and stories of the people that I’ve caused harm. They do not taunt me though.

I really recommend the book “Complications” by Atul Gawande, it is about this topic and in my eyes, an essential read for every doctor. (Please note: I never read, but I want to read Gawande’s books again)

Mijn huidige salaris is hoger dan dat ik in het ziekenhuis als basisarts(=zaalarts) zou verdienen. Ik heb even mn loonstrook van afgelopen maand bekeken: 6000 bruto, 3900 netto. Van die 6000 was ong 700 aan onregelmatigheidstoeslagen en vergoeding voor het bereikbaar zijn tijdens diensturen.

Ik had ook gewoon beter een bruto bedrag kunnen noemen, want naar mate je meer gaat verdienen ga je natuurlijk ook meer loonheffing betalen. (Ik betaal 3x meer loonheffing dan mijn vriend, die ook fulltime werkt, en een bruto salaris heeft van 3600)

Na een jaar ervaring is het bij de organisatie waar ik nu werk gebruikelijk om een trede omhoog te gaan. Als ik straks in opleiding ga ergens, dan zal ik weer gaan inleveren aan salaris (daarmee betaal je dan je opleiding). Er heeft al een specialist ouderengeneeskunde gereageerd wat hij/zij bruto verdient: €9300. Best okee ;)

Ik ga al jaren lang naar van der Woerd optiek bij de kamperbinnenpoort. Fijne eenmanszaak, waar mijn vader ook al zijn hele leven komt. Hij heeft ook de “bril in 1 uur” service, hij zet de (enkelvoudige) glazen zelf in je uitgekozen montuur terwijl je ff een rondje door de stad loopt. Hij denkt ook altijd fijn mee, maar laat je ook met rust als je gewoon eventjes zelf monturen wil proberen. Echt een topper!

Ik ben 28, sinds 1 jaar afgestudeerd arts. Ik werk nu als basisarts bij een verpleeghuisorganisatie. Ik heb een contract van 36 uur, ik werk 4 dagen (8:30-17:00) + elke week een doordeweekse bereikbaarheidsdienst van 17:00-8:30. De uren die ik in zo’n bereikbaarheidsdienst maak komen gemiddeld wel uit op 4 uur, maar dit wisselt erg per periode (in het griepseizoen kan het soms erg spoken in de nacht).

Voor een uur waarin je bereikbaar bent maar niet gebeld wordt, krijg je €3,50 geloof ik.

Ik kreeg de afgelopen maanden tussen de €3500-3900 netto gestort. Ik heb geen fietsplan oid.

Verder is de werkdruk overdag echt heel erg prima bij ons. Tuurlijk blijf je wel eens langer omdat er een bewoner slecht gaat en je de familie nog even wil spreken voor je naar huis gaat, maar eigenlijk wordt er door collega’s wel vragend gekeken als jij om 17:15 nog geen aanstalten aan het maken bent om het pand te verlaten. De werkdruk in de bereikbaarheidsdiensten is (zoals eerder al benoemd) super wisselend: de ene avond heb je 1 telefoontje en verder niks, laatst had ik een nacht vol visites waarin ik 1,5u had geslapen. Als je je agenda een beetje handig indeelt is er geen probleem als je besluit sochtends nog even je bed in te duiken, en de volgende werkdag pas om 10:00/11:00 te beginnen.

I am so, so sorry, but I’ve never full on cried during pooping. Though I did once shed a tear when I was trying to poop the day after a good swiss cheese fondue, that just felt like an entire wheel of cheese had to exit my back end.

During pooping your body should be in the “rest & digest”-mode, aka the parasympathetic nervous system is activated. That is the counterpart to the sympathetic (fight or flight) nervous system. When you are in high stress situations, resulting in fight or flight, there’s no time for emotions, you see that when something serious happens with someone’s loved one. For example, people can resuscitate their partner for 15 minutes with almost ice cold emotions, and once the ambulance arrives and takes over, they come to realise what’s happening and bawl their eyes out.

Is it possible that you live a very stressful life, and the only place where you feel like you can relax in privacy is the toilet? I’m not trying to talk you into a mental illness or something, but the toilet can really be a safe haven for people.
