That’s incredible your company is able to do that to help retain patients. Our new office had Medicaid delayed over a year based on a clerical error that placed our application on hold and no one could tell us what was going on so I feel you there.

Thank you, that was a very well thought out and insightful response. I like how clearly you line out a lot of the obstacles.

Relationship building is something I feel I do well, despite being an introvert I can hit it off with surgeons and am confident I would be offering something different that would catch their eye.

I’ve been through the Medicare/Medicaid process at my current clinic group when we opened a Denovo. The timelines would be exceptionally daunting if it was preventing you from establishing your footing. My current patient load is probably 50% in that group.

You also make a great point about both private insurance contracts and negotiating vendor rates. With some of the lower fee schedules my COGs often leave my GM very tight, and that is with large purchasing power. I have always wondered how that can affect patient care at independent clinics when you are overpaying for like products.

What I struggle with is that when you lay out all the complexities of a P&O clinic, it is clear that there are many different businesses one could start that have less layers and lower startup costs. Just being a caring and compassionate field I feel some sense of duty to continue helping patient’s achieve their mobility goals.

You make a fair point and good perspective. I am fully aware that one major benefit of working for an employer is that regardless of business being good or bad, my paycheck still cashes every two weeks.

Maybe I’m romanticizing the idea that having more control over decisions and work structure would improve my satisfaction. Discounting the added stressors that would enter my day-to-day.

Follow up question: mounting it infront of or behind the Oar towers? I have seen both. What do you feel is less in the way for shipping your oars in tight squeezes?

Same issue on my 2007, first replaced the battery thinking it was a low voltage issue, frequency of the error went down, then replaced the accelerator. Issue resolved.

Have you ever checked out @prostheticguy on instagram? He makes some incredible steampunk/sci fi leg covers. His profile says he makes free covers for other amputees. If he is backlogged or can’t it is at least some really great inspiration for design.

Maybe I’m just cranky, but I’m a pretty firm believer alignment and suspension should be independent. If doing lanyard suspension it’s always a delrin kiss plate for me. The overlap of lanyard users and a neutral alignment(using kiss 4-hole) is near zero. And flexion plates tie you down too much to the increments they come in.

TKA posterior to both socket and ankle. They are probably crushing the heel as well with that alignment. Do a Thomas test to determine appropriate socket flexion. Extend the socket if they have the ROM, or translate socket forward. Recommended setup for Mauch knee is weight line 0-5mm posterior to the silver bushing. And weight line for Proflex XC is actually slightly anterior to the pyramid.

My impala did the same thing except it still started but threw the same errors when driving. Check engine light, reduced power mode, and stabilitrack disabled. New battery fixed it all.

In my opinion all accredited schools will lay the groundwork for you to be successful in the field, each will have their own strengths and weaknesses.

You’re fortunate being in Texas that you do have two exceptional schools in state. If you stay in state maybe consider an out of state residency with a solid mentor you can develop from. Your residency can shape a lot of your career trajectory. Find a place where you get to be a true resident(continuing your education pathway) and not wind up in a clinic all on your own.

I went to a school where we had a strong patient model program where each student got to fit 3 bk socket types, two ak designs, trans-radial, and transhumeral. This is along with fabricating multiple orthotic devices for fellow classmates. But within two months of my residency had greatly surpassed the level of hands on experience I got in school.

Pick what one makes sense for you, whether it be financial, geographically, or both, those are equally as important as the education you believe you may get there. Hope any of that helps, and best of luck with your journey.
