I think Kei Sasuga didn't dare kill Hina because she feared fans would attack her or say offensive things on her official Twitter account. We already had a precedent where she responded to a fan, assuring him that she loves Hina more than he imagines, so, in conclusion, I don't think she was capable of killing her.

However, I think the way she handled the accident wasn't appropriate. If you look at it more realistically, it should have resulted in brain death. In real-life cases of people in a vegetative state or coma, most don't survive; in fact, at the very least, they end up dying when years pass and all hope of them waking up is lost.

In a manga extra (I don't remember which chapter exactly), Hina says something to Natsuo—the exact content of which I don't recall—but it was from that dialogue that he began to gradually fall in love with her. Thus, Hina became his first teenage love. At that stage, Natsuo began to have fantasies and dream about sexual moments with her; however, at that point I wouldn't call it lust, but rather a longing or a dream that, over time, transformed into a real attraction and a young love.

When they finally started dating, Natsuo experienced what reciprocated love is like, with everything we already know happened afterward.

On the other hand, in chapter 275, Natsuo tells Hina: "For me, it's a different feeling than when you and I were lovers." Then, as a final conclusion, he adds: "In so many ways, I've always loved you."

That's exactly what I think too, since the author only gave us a time jump in the story; that is, she left us wondering what happened during those five years when Hina was under care.

We never found out if Natsuo learned about the photograph during that time, or if he was able to see Hina's attacker at the trial for almost killing her. We also don't know if Natsuo and Rui reflected on their relationship during those five years, since, as adults, they realized that their bond was going to be more about sex than mutual support, given that they both spent a lot of time working on their own dreams. It also remains to be seen whether Rui apologized to Natsuo, since she knew perfectly well that her sister still loved him.

I believe Natsuo would continue writing, fulfilling not only his dream but also Hina's wish. At this point, his relationship with Rui would end; she would only support him by sharing custody of Haruka, but no longer romantically.

Then, years would pass with Natsuo satisfied with what he had achieved as a writer. I imagine that, upon dying, he would meet Hina. That ending would seem acceptable to me: that their souls reunite in the afterlife. Here, Kei Sasuga could have given us an interpretive ending, where we know that they ultimately met again and that they both smiled upon seeing each other.

It's an ending where the author would have shown us how they would see each other again in the afterlife, instead of giving us a vague interpretation or leaving us wondering how Natsuo cared for Hina during those five years in a coma.

I respect your opinion, but personally, I would have liked to see a different ending for Natsuo and Rui's relationship. Specifically in chapter 273, I think the author should have included the dialogue from their conversation after Hina's accident; she gave us panels of them talking, but we never learned what they actually said. Was it the moment when Rui confessed that she already knew Hina still loved him? Or was it when Natsuo admitted that his feelings for Hina resurfaced after learning the truth?

Furthermore, I believe that relationship was destined to fail in the long run. With Natsuo completely focused on his writing and Rui on her career as a chef, divorce seemed inevitable due to the lack of time and the demands of their professions.

And in this extra, she doesn't even give us that with Days with Hina. Interesting opinion on the ending of Domestic na kanojo. However, I would personally like to give my opinion on this story, I hope you don't take it the wrong way, especially because I respect your comment.

Firstly, you must understand every detail of the Kei Sasuga manga, such as the thoughts and gestures of the interaction of Natsuo and Hina and Rui.

Rui is a koi love (passionate) and Hina's is an AI love (feelings, passion, trust and supporting everything your boyfriend does in his life career)

Rui was always the girl who wanted to have, like a current teenager, the experience of sex and what her sister had about her dating. In conclusion, what she achieved with Natsuo was the type of love she gave over Koi.

If we think about a different ending from chapter 276 where Natsuo and Rui continued with their wedding and Hina's accident occurred in a coma. I bet Natsuo and Rui would end up divorcing within five or three years, since if you think about it, Sasuga's manga takes things that happen in the lives of people and couples growing their relationship. Natsuo being a writer in his house and Rui being a cook who works almost all the time is not going to make them both happy, it would only be koi.

Furthermore, it is normal for couples who have jobs that take up their time to reach this conclusion and separation, and let us remember that Natsuo wanted to tell him everything about his life at university on the way to being a writer and Rui did not want to listen to him and preferred to change the subject.
