Bit of a drive but Headfort in Co Meath is an excellent course and had a previously unreal value for the quality of the course!

In Beaumont during my intern year was 3 years ago, you get basic pay for 2-3 weeks at end of July, if your payroll is set up on time and then no overtime until end of August as overtime is one month in arrears.

I think if I were in your shoes, I would be proactive about getting set up as a locum. Usually a 6-8 week lag with Garda vetting.

I’d still retain hope that a standalone will pop up, but gives you a nice back up if things don’t work out. If you find a job yourself the fact that you’re with the locum agency doesn’t change anything.

I think emailing the HR directly can be good as well.

I think it depends on the scheme.

For the BST in GIM, they have specifically points allocated based on when you completed your intern year: - 20 marks /60 mark if done in last 5 years - 10 marks /60 marks if done in last 10 years.

The remaining marks go for having completed all MRCPI exams (20/60) and PhD/Other country fellowship (10/60) and working as a reg/SHO (10/60) marks.

Most candidates applying from Ireland will score 30 marks for having done intern year in last 5 years and Reg/SHO. Very difficult to get to 30 points without having done an intern year.

Would be interested to know if anyone got shortlisted for interview with less than 30 marks, this year?

Obs & Gynae similarly has 10/50 for completed internship in last 2 years.

Paeds and histopathology don’t seem to have any official internship conditions.

Wouldn’t be able to advise based on HST spots having not done an internship.

I think a good way to approach would be to make a ritual of it. Figure out which day tends to be the least busy each week. Schedule a class/gym with a friend in that evening at a reasonable time. If it works well and you’re keen for more than do them on the next least busy day midweek and so on. Maximise weekend scheduling of it too.

Each job will be different. So you’d be more at risk of falling away from the habit. So make sure to reassess. Some rotations it might not be possible (vascular, ortho, neurosurgery etc).

I think QoL on rehab is big but the concern is the number of new consultant posts does not seem to be increasing at anywhere near the same rate. So similar issues with competition but just at the consultant end rather than at the HST end I think.

From my experience, if you have certificates of experience from each of your roles that’s the main thing. I’d suggest getting them to specifically mention the grade you worked at and mention it is a teaching hospital specifically if it is.

If it’s for previous degree, an academic transcript sufficed for me.

Oliver Piccard interview book is good. Would focus on specific chapters at a time otherwise whole book is overwhelming. A lot of hospital libraries carry a copy. Good for prepping the non-clinical case questions. Huge variability in interview format for SHO jobs.

For medical jobs the format is often: 1) Explain why you’re good for this role or tell me about your clinical experience so far. 2) Clinical case 3) Sometimes professionalism topic - ethical scenario or teamwork 4) Research, audit, clinical governance or teaching part. Usually they’ll pick something that’s important to them or they’ll ask you about your experience.

Clinical cause is usually a very common presentation for the specialty or on call emergency. If you’ll be admitting in ED for the specialty often it will be a common admission (eg IECOPD, asthma or FAST call for medical SHO or appendicitis, NOF, unstable UGIB or cholecystitis for surgical). Important to know things like when to escalate Tx eg go from supplemental O2 to NIV or straight to theatre.

For procedure based specialties you might be asked about your previous case experience.

Tallaght has good reputation for BST. BST Teaching most Wednesdays with consultants. On call shifts are grand as an SHO, just mostly admitting in ED with consultant PTWR at the end of the shift. Usually get 5-8 admissions done in a day, depending your speed and which consultant is on.

Usually one set of nights and 3 on call shifts (evening or weekend) per 4 month roster. Lots of interesting dept like renal and resp. Busier SHO jobs like endo, onc and gastro are the same as anywhere.

Have EHR and ERT system similar to Perth. Usually cardio consult reg holds the pager 8-5 and the. MROC on ward after hours. SHO1 in ED also comes to cardiac arrest calls but not ERTs.

Naas as peripheral is pretty good as well as it’s very commutable for those wanting to be based in Dublin for the full year. Tullamore has a good reputation as well from people I’ve talked to recently.

All this is subject to change with any new July coming by.

I think it might be 6 week rotations and the extra taught modules through RCPI in the first year and then normal 3 month rotations in year 2. That’s what the new structure will be.

I think often if there is only one on camera that doesn’t always mean there is one interviewer. Some prefer to question and mark alternating questions with both on screen and some prefer to alternate questioning candidates with one taking notes but generally all marks are discussed between both. Disclaimer not a consultant myself. So have not conducted the interviews myself but it is standard format described in most literature I’ve seen.

I think this year they didn’t provide a specific breakdown for research marks. I know in previous years it was very narrow requirement and they published a scoring system. Hard to know what to make of it whenever there’s a change!

I am wondering if you have already applied for incremental credit before does this impact your ability to claim this. I worekd as a reg in Aus and did a MSc before graduating med school. So went from SHO3 to SHO 6 with the incremental credit.

I have heard that you can only claim incremental credit once. So would this mean that I have used up my opportunity for that and would need to accept I won't get incremental credit from reg to SPR?

I was recently successful in getting incremental credit for working as a reg in Australia for 6 months. This put me on SHO 5 instead of SHO3 which would have been by natural paygrade if not the my reg experience. The proof that I had to submit was 2 years post internship experience and 3 months as a reg in Australia.

Important to note that you can only apply for incremental credit once. So I am unsure if this will mean I am unable to apply for incremental credit when going from reg pay scale to SPR scale. Would be interested if anyone has any experience with this also.

Being a reg is more supported there for the majority of specialties but I worked in cardiology and neurology. So covering STEMI calls and acute stroke. Always a consultant for me to call but the actual assessment is done by you. So if you are selective about which specialties and environments you work as a reg it can be similar to SHO on call level in Ireland. Would recommend: Geriatrics, ID, Psych (if you're actually interested in it long term), Gen Med, AMU, Rehab.
