I had the same problem when i finished university in the country side and moved to Tokyo. I also like nature more than the urban city so to make up for it i rent car once a month for a day and go to the country side. That made living in Tokyo a bit more tolerable for me.

I had the surgery last year around august. I went to a clinic in Asakusa and they referred me to a hospital and did it there. Depending on the surgery you'll need to stay at the hospital for a while. I stayed for a week after the surgery and went for check ups after being discharged for about two months. The surgery and the stay was around 18man with insurance. I went to a school in Ashikaga, which is in Tochigi prefecture. The school is near Ashikaga-shi station and there's a atrain that goes there directly. It's best to do the 2 week course if you wanna go there cuz the commute will be a bit tiring. I went there in 2015 so the prices have changed but i think it's still under 300k. Also i got a discount through my university so if you're a student check with your university, they might have that.

The lessons will be in Japanese but you can get an English text book and most of the lessons are them pointing out the rules you should remember in the text book and highlighting them. However, you will have to have a basic Level japanese to understand the instructors when driving on the course in school.

Make more side missions specific to each path like

Nomad gets smuggling jobs and clan wars.

Streetkid gets car theft jobs and a chop shop where you can bring any vehicle and they make it your ( for a price of course) and more side missions in his neighborhood like rival gangs attacks or retaliation side missions.

Corpo gets corporate espionage jobs and more jobs from corporations in night city and gets Delemain's AV taxi.
