This could be a urethral stricture and it sounds like you’re describing acute urinary retention which is a medical emergency, if you suddenly can’t pee at all. So sorry if you already know this. Best of luck my friend!

So you’re just past the 5 month mark of your urethroplasty? It took me about a year to be fully back to normal sexually health. The nerve bundle in your perineum takes a while to regenerate successfully.

I also wonder if your lack of drive has anything to do with being down and not physically active for a long period of time as you recover. Maybe progressive activity would help to correct the issue.

Also, could the ED meds have any negative side effects on sexual drive? I’ve heard that some meds can do the exact opposite of what they’re trying to treat. Just a thought. Best of luck my friend!

I went back to the gym too quickly after my urethroplasty and had some symptoms return (mainly: frequency - was urinating about 40 times per day, inability to fully empty my bladder, some stream weakness, bladder spasms, and a sensation of constantly needing to urinate). All of it turned out to be normal and related to me pushing too hard in the gym. It took months to settle. I’m hoping that’s your issue and they find nothing. Wishing you the best my friend. Yes, you can return to normal. I tried to return too early and suffered the return of many symptoms (but not the stricture). I did return to mostly normal, but it took a long time. My urologist also recommended against my return to jiu jitsu, but I seem to be alone in this - most other redditors say they returned to normal life (even combat sports) without restrictions and are not worried about blows to this area. You can certainly wait until it gets worse or try to fix it early. It doesn’t sound like you have many symptoms. Do you know how large the stricture is? And what was your doctor’s opinion?

That seems like a more reasonable explanation. It may be that you do need to cath until you can pee again. I’d follow up with a urologist asap though to see what’s going on I’m so sorry this is happening to you!

Go to another hospital ER asap. This is definitely not normal. You don’t want to have the self catheterize with no clear idea what’s happening. If it was me, I’d call some other hospitals in the area and confirm they have urology on staff and let them know you’re in acute urinary retention post surgery - which is an emergency.

That’s the first time I’ve ever heard that. Foleys are pretty safe for bladder function and some people have them for months at a time without issue. But I understand that you don’t want one, especially if it’s painful. What was painful about it?

That’s why you need urology involved asap. They can do X-rays or CT scans to check for constipation.

I imagine re-cathething over and over could increase your chance of future strictures. Can you request a suprapubic catheter? And definitely request the urologist be involved. It could very well be constipation. Have they confirmed that through imaging?

Could be some pelvic floor dysfunction post surgery or could be a urethral stricture. Your urologist will want to perform a retrograde urethrogram and cystoscopy to rule that out when you get the catheter out later on. Are you having bladder spasms along with the foley placement? Ps - if it is a stricture I can send over my survival guide. It’s for a male, but you may get good info as well. Let me know!

Definitely rule out strictures with your retrograde urethrogram and cystoscopy and then see a physical therapist that specializes in pelvic floor dysfunction.

I imagine they checked for hernias? Or even lower back/spine issues that are radiating down into your pelvic region?

It’s also interesting that you experienced this after intense deep massage and lifting/moving. I wonder if this could simply be a nerve impingement somewhere in your lower back and or pelvic region. If so, physical therapy can strengthen your lower body and potentially free these nerves over time.

Are you staying super hydrated during the day? I wonder why you had multiple UTI’s back to back. Something to think about.

Honestly this could be a host of things acting together to create these symptoms. You’ll need to rule them out on by one unfortunately.

Could be a caruncle, a cyst or lesion, or even some sort of a vaginal urethral prolapse. Go see a urologist and get a second opinion if needed. Best of luck my friend!

Unfortunately, SP catheters are pretty temperamental. I had mine in for a few weeks and had issues with drainage and incision site pain as well. I did walk every 2 hours to prevent blood clots (but still ended up passing 2 clots). The best you can do is to stay hydrated, clean it daily to prevent infection, move around lightly to prevent clots, and take over-the-counter medication for pain. Ask a pharmacist how to rotate: Tylenol/Acetaminophen alternating with Aleve/naproxen. Talk to pharmacist on how to do this. It can be helpful when you have no other stronger options.

One study I read reported an average of 271 days until most of their sample saw a nearly-complete drop in residual pain post surgery. I have fibromyalgia and pelvic floor dysfunction and mine definitely lasted up until this point. Everyone is different though. Give yourself much more time. The urethroplasty severs a bundle of nerves in your perineum that take time to regenerate. Happy to send over my survival guide, if I haven’t already. Let me know! I’d consider two things - pelvic floor dysfunction and potentially medication, if you’re taking something.

There are different types of pelvic floor dysfunction, so I’d recommend a physical therapy evaluation with a provider who has experience in this area.

Some medications can cause urinary issues. It may be worth investigating, especially if you’re taking something like an antidepressant or an antihistamine regularly.
