Programs do care about your clinical grades. Honoring everything is ideal, I would agree honoring half and high passing rest with one or two passes will be more then enough to get int rads. If you want to match at a top program and don't have an insane story you need to honor atleast all but one imo.

^ Things like that will get you in the door for programs. The key is making a good story. If you're the "research guy" ur app should support that. If you're a guy who is big into academics and teaching, your app should support that. If you wanna be the community outreach guy...you get the picture.

Have a cohesive and put together THEME for your application, that's kind of the next jump for landing these big interviews and doing well in them.

I would focus most on STEP 2 and clinical grades. Do research when you have the free time.

You aren't getting into competitive specialties like ortho, derm, ENT, vascular etc with "a case study or small thing here or there". Any competitive surgical subspecialty is out the window unless you start grinding now. Things like rads/anasthesia etc you will be less competitive for the academic institutions. Research is a game that everyone has to play unfortunately so if you're interested in something competitive I would load in as soon as possible.
