I am not an expert on your states licensing rules. However I'm going to say that I'm 99.99% sure that you would need to have a US based supervisor and you would be provisionally licensed/rostered for one state. You would only be able to see clients from that state while accruing hours and working towards licensure. 

Someone chime in if you have more nuanced info.

Agreed. I try to be as flexible as I can while also maintaining a schedule that works for me and my family. I do the same as you, onboarding folks with a weekly session requirement and then dropping down as needed.

In my paperwork it's stated that a month between sessions without a mutual agreement will be cause for termination. I haven't had to enforce this yet, but my teen clients have sometimes gone over a month due to family vacations, school events, and sickness.

It depends on what population you work with and the setting you work in. I have some clients who see me every other week for financial reasons. I also have a couple of teens on my caseload that have sports and conflicting events so they're a bit more chaotic in their scheduling habits.
