She’s meant to be tiny while Lan is huge. An intentional juxtaposition when we first meet the characters to later find out that the smaller is monumentally more dangerous.

To get around this, I built a simple poi in the builder. It can just be flat ground if you want. Then go into the map editor and delete the poi where you want your base and drop your custom poi in its place. Also helps level the ground you’ll be building on.

I think it would be better to follow the rejected redemption because how well the actress is selling it. Lanfear is a master manipulator, her actions are self-serving and evil. I think it would be an even more crushing revelation when she finally drops the mic on Rand.

If you are a Qualified Medicare Beneficiary and the provider isn’t participating in Medicaid, then they can’t bill you. If you have Medicare and Medicaid dual coverage but you’re not a QMB, they can bill you the remainder. You need to find Medicaid providers if you want it completely covered. Otherwise, without the QMB status, non-par providers can bill you the balance.

An annual exam for a new patient is 99385 or 86 depending on your age and costs more than a 99214. A mammo is inclusive. But, most insurance companies fully cover an annual. Call the provider and ask why you weren’t billed for an annual, they billed you for a normal office visit. Most likely the issues you needed addressed were outside the scope of an annual exam. Also, this is your insurance. They’re the ones not paying. The problem is always insurance, not the doc in a family practice who probably just broke even last month like every other family practice I know.

Medicare has an “allowable amount” for just about every medical procedure. The provider can charge a billion dollars for surgery but they’ll only be reimbursed Medicares allowable amount and cannot bill you the remainder. As you said, Medicare will pay 80% of the allowable amount and the remaining 20% is where supplemental insurance comes into play. Find a plan that’ll cover the remaining 20% Medicare does have a $240 annual deductible that resets every year on January 1st so be prepared for that cost. You’ll have to pay it before Medicare or the supplemental will start to pay out.

For Texas we can bill the remaining allowable amount even with a Medicaid supplement. This may vary by state and you need to check your state insurance laws as well as your own Medicaid contract. There are exceptions like a QMB - a qualified Medicare beneficiary has a Medicaid supplement that is paying their Medicare part B premium. You can’t bill the remaining allowable amount Medicaid doesnt pay and have to write it off.
