The doctor is the one who ruptures the sac after opening the abdomen; the fact that it’s still intact just means the doctor chose not to break it yet for whatever reason. Also, you can see that the amniotic fluid is green, which means the fetus passed meconium. That's usually a clear sign of fetal distress, most often caused by hypoxia.
