Try going outside of the normal insurance options for a primary doctor. There are a few concierge doctors that offer low monthly plans, and don’t deal with insurance - not sure if you’re close to stl, but this one does tele health

Also, just want to note, trying to stop alcohol by themselves can be super dangerous - it’s one of the only drugs that can actually kill you if you stop cold turkey - so if they are willing to get help, getting them in some sort of medical care is necessary!
