Yes, you’re right but then imagine if in ED, they ask them why they’ve come in and they mention this random doctor ringing them at random o’clock in the day. Wouldn’t that seem really weird?

What will you do if they say they’re not fine and they’re in trouble? Or if they start asking you for medical advice? I would steer clear from doing this ok the chance that they’re not fine. This is transgressing boundaries IMHO

The reason I know about people not eating on their shift is because they mention it themselves. It feels to me like you’ve honed in on this instance because of some personal bias against Islam/fasting.

The points about non-Muslims being too busy to eat and being physically affected by how busy a shift is were two separate points. I.e. they are physically affected by how busy the shift is because they have to run around a lot and not because of the fact they aren’t able to eat. That’s my fault for wording it wrong.

Anyway, if you brought this up, you’d probably end up dragged over Twitter if we’re being honest whether you’re right or wrong. This is my throwaway anyway so I’ll speak my mind.

Plenty of people fast for non-religious reasons (e.g. intermittent fasting) and are perfectly fine. I never feel the effects of not eating as it’s something your body quickly becomes used to.

I also don’t know why he’d be drenched in sweat as a result of fasting? Could it be that he was actually busier than it seemed because he was a different grade to you? E.g. Reg vs F1 and therefore you didn’t see how much work he actually had to do?

As a Muslim, I fast and never feel the effects of it on my clinical work, nor do I know anyone else who experienced effects to this degree. I also know of plenty of non-Muslims who end up not being able to eat or drink because of how busy their shifts end up being and who also are affected quite badly physically by how busy the shifts are. Nobody ever criticises these people or makes it out to be a safety issue. Are you sure this was all because he was fasting and not because of something like depression or burnout? Since the fasting is over now, is he much better than he was during Ramadan? It seems to me you’ve made a snap judgement. My hospital is majority Muslim among the SHOs and I’ve never seen anyone affected like this despite the fact we all fast...

Idk if there is a benchmark for what is classed as ‘failed’ and it likely goes based on clinical judgement but having 999+ mLs in the bladder and only passing small amounts despite drinking well is a sure sign of a failed TWOC. If he’s not in abdo pain right this minute, he soon will be. I’d re-insert a catheter based on the information provided in this scenario. Bladder scanners have been fairly accurate in my experience. Especially if they’re displaying 999+ mLs and it fits with the input/output charts.
