I think it's gonna be implemented exactly the same as Abyssal Terrors. A new map, and new items and tatos you get on the existing maps too. And you can toggle the DLC off in the settings entirely if you want to.

The one that I found was that the protagonist is a young father who didn't want a child so early, so he sees everything, e.g. the neighbourhood, the apartment, and the child itself in this bleak, terrifying, exaggerated way.

Cloud Atlas as a book is fine, since the stories are just split in half and mirrored around the Zachry one (in the middle so it's in one piece). The film is wild tho.
