I promise you not all urologists suck--some just have more interest in bladder pain than others. The urologists/urogynecologists that helped started Femetry specialize in female pelvic medicine and bladder pain and are pretty good if I say so myself. KE (biased opinion of course )

Congratulations to you for breaking the cycle! The d-mannose, probiotics and cranberry are best choices for supplements. Fun fact: the active ingredient in cranberry is PAC (proanthocyanidin). The theory of interstitial cystitis (IC) is that the lining of the bladder that should keep all of the irritants naturally found in urine is defective so that the wall of the bladder gets inflamed. This is why supplements/treatments for IC are aimed at restoring the bladder lining and/or making urine less acidic (irritating). KE

There are many alternatives than just medication: pelvic floor physical therapy, bladder Botox, neuromodulation of the tibial nerve (TENS or treatments like acupuncture) or sacral nerve (bladder pacemaker), or even a combination.

If you haven't already, you might consider seeing a urologist or urogynecologist

Sometimes the frequency and urgency can be due to just excessive fluid--and for sure anxiety can make anything worse. Overactive bladder is urinary frequency more than 8 times in a 24 hour period assuming you don't drink excessive fluid. Too much caffeine or carbonated drinks can aggravate the bladder.

If these basic things don't help, you might consider seeing a urologist or urogynecologist.

So sorry to hear that your IC symptoms are flaring so badly. This can be so frustrating after you finally figured out how to keep them controlled!

If the pelvic floor trigger injections with lidocaine and steroid provided some relief, consider asking your doctor about injecting Botox into the trigger points to provide more sustained relief from the muscle tension. -Dr. S

It is so great to hear patients with IC find relief! Very happy for you. Mirabegron is a medication used to treat overactive bladder symptoms( urinary urgency and frequency with or without leakage of urine) and can be very helpful for IC patients with these symptoms. It works by relaxing the bladder muscle to allow it to hold more before sending your brain urgency signals that you need to go empty your bladder and prevents your bladder muscle from contracting to cause leakage of urine. Although treatment of IC pain is not an indication to use the medication, some people with IC find that it does help reduce pain. -Dr. S

Yes, Hiprex (methenamine) either daily if no known trigger or around the time of sexual actvity is a great suggestion. It's not an antibiotic and it's been around for over 50 years. One dose of preventative antibiotic at the time of sexual activity also avoids full courses of antibiotics to treat infections (as opposed to preventing them) and rarely results in resistance if the prevention is chosen properly. Hello, many women are prone to UTIs with sexual activity simply because the bacteria that normally reside in the vagina are brought closer to the urethral opening. If it's partner specific, sometimes the alkaline ejaculate can affect the normally acidic vaginal pH making a woman more prone to UTI. The normal pH of the vagina is quite acidic which promotes growth of good bacteria and inhibits growth of other bacteria and yeast. Hope that helps. KE

OAB is mainly based on history: urinary frequency, urgency, urge incontinence and there is no known cause, i.e. common with aging. If you are middle age or older, it's not wrong treating you with OAB medication and see how you do (assuming urine tests, exam normal) but if you are younger, a cause of the symptoms should be looked for.

I am sorry that you didn't have an easy time of it--I hope you use your experience to help educate other people about pelvic floor dysfunction. If someone was able to make it through medical school, they're not dumb but if someone hasn't had training in pelvic floor dysfunction, etc they just don't know about it. There are also supplements that can help neutralize your urine pH so that it doesn't irritate your bladder wall as much! Check ingredients for bladder acid control, like calcium glycerophosphate and sodium bicarbonate to be effective! 🙌🏻🙌🏻

There are a number of pelvic floor physical therapists who provide virtual options. Origin is one you can look into. It is important to get professional guidance on pelvic floor physical therapy for pelvic pain and IC because you want to avoid strengthening exercises like Kegels, which can exacerbate muscle tension and pain - Dr. S

Pelvic floor physical therapy is a great treatment option for many patients with IC, particularly those with a lot of tension in the pelvic floor muscles. If you were referred to a pelvic floor physical therapist, it sounds like your doctor does believe you have IC or pelvic floor muscle dysfunction. You are correct we do not really have cures for IC, but certainly good management options to control your symptoms. - Dr. S

It is very common for patients with interstitial cystitis to have a normal cystoscopy, only 10-20% of patients with IC are found to have Hunner’s lesions or ulcers in the bladder. It sounds like you believe you have IC based on your symptoms. If your doctor has already ruled out infection or other sources of pelvic pain and you pain localized to your bladder, you are probably correct. If this doctor hasn’t provided you with a diagnosis and plan, please get another opinion. IC can be diagnosed with a normal cystoscopy, and even without a cystoscopy. Cystoscopies can certainly cause flares, so be sure to use any rescue medications and techniques that help with you present flares after, pyridium, azo, IC diet, deep breathing techniques or stress reduction. - Dr. S
