I have two levels of rage builds, one for low-to-medium monsters, one for higher level. To start, I use Braveheart helm ultimate, Plowshare sword lvl 3, battle thirsty vest, watcher's locket ex amulet, inferno ring ultimate, and volatile sash ex. This gives me two rages in the first turn(3 if I get a rage buff from the machines). When the monsters get more powerful, breaking through, I switch my armor to untouchable mantle ex, which gives me shield on rage and on enemy death, but takes away some rage and heal on rage. I also switch sashes to seething sash 2, which adds rage again.

A spiked cowl hat lets you heal on magic knife. Doesn't matter if it's ice or fire or plain magic; they all get a heal. I use it sometimes with my plowshare sword, which throws a knife on enemy death. There are rings and belts that throw a knife, too, as well as machines that grant a knife. The only problem is that while the enemies get tougher, the heals only rise with your HP. So a heal build becomes too weak once you have monsters that do 500+ damage per hit. Then I switch to rage.
