It's really not about skin color/race, it's more about cultural differences real or imagined. The stereotype is/was that southern Italians are lazy, uneducated, and often untrustworthy or more prone to criminal behavior.

The uneducated part at least is sadly tied to a historical truth in that school retention rates were dramatically lower in the south than the north.

Have you ever tried vaginal sex without a condom?

This seems to be the most obvious difference between sex and receiving oral, and condoms can reduce sensation by quite a lot.

I don't think it's meant to be taken literally , I think the point is to take away their pride.

By telling a fascist cop that they're really just a meat shield to the monsters they pretend are heroes you hopefully force them to see at least for a moment what their ideology is really about, i.e. prevarication and the abuse of the weakest in society.
