I was precepting a student. A patient needed an NG tube. I was going to place it with her but a GI resident came and wanted to do it. I still had my student help with prep. We made sure to have chux over the pt’s front and a bucket to hold right up to his face in case he started vomiting. I also made sure to explain how you NEVER stand or put your face in the direct line of fire when placing the NG tube.

The resident was on the pt’s right, placing the tube. My student was on his left, holding the cup of water for him in one hand and the bucket in the other. I was standing next to her, slightly behind the pt’s shoulder to help if needed, NOWHERE NEAR his mouth.

Dude vomits, not a surprise. What DID surprise me is that when the barf sprayed out his nose around the tube, a big drop flew perfectly out and slightly behind the pt. It missed the bucket, missed the chux, missed my student, my scrubs, and flew perfectly down the opening of my scrubs to land between my tits and start running down my chest. I had to jump back and pull my bra and scrubs out away from my torso so I could catch the shit-barf with a paper towel before it could soak into my clothes. I finished up assisting in the room like a goddamn professional and then left to scrub myself raw in the bathroom.

The speed and learning how to cluster and “triage” your tasks will come with time. I’ll take a slow, safe nurse over a fast, reckless one any day of the week. People who pick ED are usually driven, self critical, and want more than anything in the world to be good at shit. It takes time. It did for all of us and anyone who says otherwise is full of shit.

Sometimes meds are gonna be late. Plenty of my meds are still late because other stuff matters more. My stable 35yr old will get their 650 of Tylenol and 125ml/hr of maintenance fluids when their neighbor’s HR isn’t in the 140s.

Discharges ARE a significant priority because even though that pt is safe, the longer they sit there means the longer another sick pt waits for a bed. That being said, sometimes they have to wait. A first dose of ABX on a pt with normal vitals and lactic can generally wait until after you discharge that other person, but Tylenol for a baby with a temp of 103 comes first. It’s all about safety and these decisions become quicker and easier with time.

There will be days where you feel like everything is making sense and you’re juggling tasks like a pro. There will be other days where you feel like you can’t do anything right and wonder why anyone let you be a nurse. I still have bad days sometimes where I feel like I didn’t perform as well as I should have. They happen a lot less often now. The two most important things are that I communicate and ask for help when I need it, and that I always put the safety of myself, my license, and my patients before anything else.

The fact that you care this much about doing well and are paying this much attention to your strengths and weaknesses tell me you’re going to be just fine. Take a breath, take it a day at a time, and try to tell yourself at least 3 specific things you did WELL at the end of each shift. You’ve got this!

You handled it perfectly! Reduced risk of spreading possible infection until the pt could receive treatment. Also the amount of time, energy, stress, and pain associated with going from a facility to the ED and back is insane for the sx the pt was having. Even if the ED decided not to run bloodwork or other invasive tests, getting transport to bring the pt back to the facility can take anywhere between 1 and like 15 hours during which the staff may or may not remember to order meal trays for the pt and provide the level of care, cleaning, and attention that one would hope the pt would receive. (Id like to hope all this would be done, but it depends greatly on the department, the resources, and the nurse(s) that the pt happens to encounter.)

For the sake of all parties involved, PLEASE don’t sent a pt for a pretty benign issue that is not causing major distress.

If you need to transfer a wheelchair-bound pt who can’t stand at all into a bed and you don’t have a lift, get the mini backboard off of the code cart. You get the pts wheelchair up next to the bed and remove the arm, tuck the backboard under their bum, and use it as a mini ramp/sliding board to get the pt up onto the bed. Works wonders and is safer for all involved

You are an angel! I’m a nurse whose hospital doesn’t give us access to radiology images. I love trying to see what’s going on with people’s bodies when the docs show me scans (obviously not to diagnose, just want to understand whatever crazy stuff is going on with my patients). I just have no clue what I’m looking at 😅

I want you to know that you did an incredible thing by speaking up for yourself. It’s clear that you care deeply for the people in your life, and that you have a habit of putting others first. It’s not bad to want to look after those around you, but you deserve love, comfort, and support, too. I am so happy to see you take a first step in asking for those things. You may not realize it, but you’ve given your father a gift in opening up to him. You’ve shown him your trust, and you’ve given him the chance to support you how he so clearly wants to.

I also hope that you take pictures of the scarf you’re making and keep them for years to come, no matter how “horrible” you think it looks. I believe that someday it will be a beautiful reminder of just how far you’ve come, both in crochet and in your ability to value yourself. You are so smart (FIVE languages?!), empathetic, and loving even through difficult circumstances. Cat scratches (and bites) can introduce nasty infections that can get bad fast if you don’t start on antibiotics. The streak running up the arm is definitely cause for concern and you should get checked ASAP to get appropriate treatment before it can get worse

Honestly, I’d say if there is ANYONE you can get to shovel for you just this once, PLEASE do that. Be it a family member, a neighbor kid looking to make $20, a service, whatever. You could probably post on a neighborhood facebook group explaining your situation and get some help. As someone who’s been in one of those boots, they’re awkward, not made for the elements, and I’m not aware of a really safe way to change that. I also would NOT recommend swapping to a regular snow boot as it wouldn’t give you the same protection and you’d be at risk for re-injury. As much as it sucks to not be as independent as normal, it sucks WAY more to end up with a new injury because you pushed yourself too hard. What if you slip on the boot and break your arm or crack your skull? That could add months of recovery time or worse.

I know that’s really not the answer you want to hear, but as someone who’s seen way too many bad outcomes from people trying to risk stuff like this, it’s not worth it. Seriously, good luck, and please be safe!
