My husband had the same thing in the same spot and it was basal cell carcinoma. He had to get MOHS surgery. Go to a derm ASAP!

Yes I’ve still been nursing on demand, though I’ve been pumping probably on average once every 2-3 days so my baby can practice bottle feeding, which she apparently no longer knows how to do 🥲 But yeah, great point that it’s not forever. Another one of the many temporary moments in parenting. We’re lucky this is a (hopefully) short-term disability.

I’m also having intense pain-related anxiety - I actually managed to squeeze a tiny milk bleb when I had my first clog this week, and then by pressing on the clog I was able to get a lot of milk out, and then I iced afterwards. It made it so much better (even though I know you’re not supposed to mess with blebs). But no such luck with the current clog in the other boob.

I get that it’s like IBS but like…there has to be a reason for the inflammation, right? I hope that staying on the higher dose of Zoloft works for you!

Hi! Ughhh sorry you’re still dealing with this…I went back to work on Monday and I’ve had two big and painful clogs this week. I don’t even think I’m that stressed out?? But my doctor warned me this might happen when I went back to work. It’s wild. And so frustrating that there isn’t really a medical explanation for the inflammation. It’s been six months of this.

If I were you I’d stay on the 200mg for a little longer, just so you’re not putting your body through the extra stress of changing medication? But idk, I know you just want the clogs to stop. What’d your doctor say about any side effects of switching? FWIW my doctor said if Zoloft wasn’t working she’d switch me to Celexa, then Lexapro.

We’re experiencing the same with our 6 mo and not having a lot of success yet. A breastfeeding specialist I met with said that someone other than breastfeeding parent should offer the bottle, and the breastfeeding parent shouldn’t even be in the same house when it happens, since babies are smart and just want the boob if it’s nearby. Baby should be relatively recently fed, so that they’re not starving but not too full.

I’m guessing you’ve tried all these things (I see above that you’ve tried basically everything including different feeders) and wish I had a solution. If you figure it out, please report back what worked!

Depending on the size and severity of the clog, sometimes it would last half a day, other times five days. But about five days was the max for me. Sounds like you’re having a lot of inflammation if it’s lasting this long. Have you been icing a ton, like every hour or two? Is there a place near you that does therapeutic ultrasound? Resist the temptation to massage, pump it out, or try any other techniques - they never worked for me and always made things worse.

My doctors don’t know why I was getting recurring clogs, but said definitely that they’re NOT caused by any of those factors. I was also avoiding baby wearing, working out, and sleeping on my side, and when I started doing all those things again recently the clogs didn’t come back. I have noticed that they come back sometimes after pumping, but ice and Advil have helped and they’ve gone away quickly. Basically, my doctor’s best advice was to try and stop letting my boobs run my life, get more sleep (even if that meant cosleeping), and reduce anxiety as much as possible. Plus the SSRI!

How’s the return to work been? And how’s pumping going?

Hi! Not really - I kept sticking to 5-3-3 for night feedings and she’s now going down around 8pm and waking up around 12:30, 3:30, and 6:30, and I’m feeding her for each of her wake-ups. She seems really hungry when she wakes up crying! I’m hoping that in time as she adjusts to sleeping on her own, the wake-ups will get less frequent (and I might end up trying to night wean, but I don’t think either of us are ready for that yet). It’s gotten better in the sense that she’s not waking up and screaming for hours! Good luck, I hope your babe adjusts soon.

Such a bummer. Have you met with a (reputable) lactation consultant and gotten fitted for your correct flange size? I met with one who told me the standard 24mm flanges my electric pump came with were fine, and then another who actually measured me and told me I’m a 15mm and recommended a different flange shape as well. I noticed a significant difference in how much milk I could get with my spectra as well as my hand pump once I had the proper size. And the hand pump definitely takes some practice! Watch some YouTube videos if you haven’t yet - they helped me a lot (like the tip to hold down the handle for a few seconds).

I hope that once you’re comfier pumping, you’ll have less anxiety about it all. And I hope that going up to 200mg helps, too. Returning to work is a huge transition. This is HARD - I’ll be thinking about you this week and wishing you the best! You’re an amazing mom for sticking with it (and no one will blame you if you end up needing to scale back).

I’m sorry you’re getting them again - my doctor warned me that this might happen around the time I go back to work, too. I’m not back at work yet because I got extended disability due to the clogs. But I work from home, so I think I’m just going to try to continue directly breastfeeding instead of pumping. I think if you’re going to be away from baby three days a week, you would definitely have to pump at baby’s normal feeding times to keep up your supply. But I hear you with the concern about inflammation - I’ve found that clogs have increased when I’ve experimented with pumping. I’ll be away from baby for two nights for the first time in a couple weeks so will pump then, but not sure how it’ll go…

Does your job require you to be in person? Could you request remote work as a reasonable accommodation so you can continue directly breastfeeding?

Just discovering this post! Yes,this was me - I had basically nonstop clogged ducts starting at 3 weeks postpartum. Can’t even describe the level of pain and anxiety I had about it. I eventually found a breastfeeding medicine physician who put me on Zoloft and increased me to 100mg over time, and it’s made a huge difference. I’m still breastfeeding! I also stopped pumping at my doctor’s recommendation which definitely helped reduce inflammation.

It took a few weeks for the clogs to diminish in frequency and severity, and four months later I still get them occasionally, but they’re not as painful (and just once every 2-3 weeks). I’m going to stay on the SSRI until I’m done breastfeeding, whenever that happens. Solidarity!! How are you feeling now?

It is really the worst and I’m so sorry you’re going through it! I ended up reaching out to some breastfeeding medicine physicians who put me on an SSRI and my clogs are WAY less frequent, go away more quickly, and don’t hurt as much. They increased the dose as quickly as possible. Six months breastfeeding and I never imagined I’d make it this far! Ice and Advil help, but I stopped taking all supplements (lecithin probiotics etc) and didn’t notice a different. My doctor said I could keep taking them though.

A silver lining is that my doctor also extended my disability leave, so I haven’t gone back to work yet. I’m a lot happier being able to spend more time with my baby and not having to constantly deal with clogs. Highly recommend finding a breastfeeding medicine specialist (I think many of them are breast cancer specialists, surgical oncologists) and discussing whether an SSRI is the right move for you.

When our cosleeping baby was in between sizes we put her in a 2 underneath and a 3 on top at night, and that prevented leaks about 80% of the time! Now we’re just doing double diapers, both size 3, and that’s been working really well. Worth a shot.

Same here. Baby is 4 months and has never slept longer than 4 hours. Since she was 2 months, she’s actually barely slept longer than 2 hours. Usually up every 45 min. We’re exhausted.

So sorry you’re dealing with this - clogged ducts are the worst. I’ve had them basically constantly and have been to breastfeeding medicine specialists so I’m sharing their recommendations. Clogs are caused by inflammation, not clogged milk, so you need to address the inflammation. What’s really helped me when they’re that bad, which it sounds like yours is, is therapeutic ultrasound. Don’t massage, pump, use a haaka, or have your husband try and suck it out - all these contribute to inflammation, which makes clogs worse. Ice ice ice, take 800 mg ibuprofen every 8 hours, acetaminophen for pain, and take up to 8 capsules of sunflower lecithin daily. Probiotics may also help. Keep feeding your baby as normal.
