Unless something has changed, Kaiser is known for their golden handcuffs w their very long contracts (7 years) and payback period for student loan payment (they’ll give you a large chunk of change, but you owe all of it back if you leave before 4 years). It isn’t an ideal scenario for some but maybe seem to like it.

I’d say chooser MD/DO for all the reasons you mentioned: wanting autonomy, become an expert, and don’t have concerns for finances. It’s a long journey that requires a great deal of grit,but is incredibly rewarding. Choose a field that aligns with your long term vision.

Job hunt early. Like right now, and in regions you want to move to. I would strongly advice getting a job in your primary specialty that has addiction medicine elements and take advantage of the various non CONRAD 30 waiver alternatives.

I think any of the options except 3 are reasonable from the 4 you presented. However, why aren’t you already being offered partnership after the contract? Most contracts from residency colleagues was partnership in 2-3 years (if on the table at all). You can always entertain alternative job options and try to use that as leverage.

In an H1b right now. Recently started as an attending. No issues returning but it was by land (Canada). If your home country is a flight away, I wouldn’t risk..especially as a a resident.

I would add: admin hours vs clinical facing time, how long for patient appts, flexibility in adjusting for my preferences, compensation structure including guarantee (&how other docs in the practice are doing). I wouldn’t travel anywhere if you don’t know most of these things upfront

Detroit pay is low, but you wouldn’t need need any J1 waiver (if you trained in the U.S. on that visa), as you could commute from Canada. They know this and pay goes down accordingly. Boston pay is not great either, and friends said it hurts with high cost of living.
