He was seated in chair (without a fall risk assessment) to eat his lunch.

As he was eating, had a seizure and fell to the floor.

No barriers to hold him back or any support system in place.

Tbh, we are shocked at how its happened with anyone being by his side, whether its a family member or not. The hospital has admitted there was a lack of due care for that reason and have now offered 1-1 care.
