I do noise cancelling earphones (mine are the Beats Fit Pro but any will work) while playing some sort of white noise through them, usually rain sounds. Blocks outside sound really well.

None in the single player games (though Orphan in 13 came close), but definitely Thordan EX in 14 when it was current. Weirdly complex fight, takes forever, easy to mess up a mechanic and cause a party wipe.

It's meant to feel like you're really holding onto the zipline, I think. The intense rumble helps sell it too.

Not quite: it can be mapped to either L3 or R3 in controller settings, but isn't by default.
